Objective:To determine the prevalence of the anti-KINECTIN antibody in Chinese aplastic anemia(AA) patients, as well as the relations between anti-KINECTIN antibody and the types, therapeutic response and prognosis of AA. Methods:By means of ELISA, The titer of anti-KINECTIN antibody of 80 AA patients was determined. The whole group includes 68 chronic AA patients and 12 severe AA patients.40 healthy adults participate in the study as normal controls. And The levels of peripheral blood lymphocyte subsets and regulatory T cells in 80 AA patients were detected with flow cytometry.Results:Among the 80 AA patients,12/80(16.25%) of them was positive in the detection of the anti-KINECTIN antibody. While, none of the normal controls(0/40) got the positive result. So, it indicates that the positive rate of anti-KINECTIN antibody in AA patients is significantly higher than that of controls (P<0.05). The positive rate of anti-KINECTIN antibody in AA patients with severe aplastic anemia (41.67%) and chronic aplastic anemia (10.29%) is significantly different. The titre of anti-KINECTIN antibody of the 12 antibody-positive cases have decreased significantly after effective treatments (P<0.05). To Compare with the normal controls; the percentage of CD4+ T cell, the ratio of CD4+/CD8+ decreased obviously in AA group (P<0.05), while the percentage of CD8+ T cell increased significantly (P<0.01). There was no significant difference in the percentage of CD3+ T cell between AA group and the normal controls (P>0.05); the positive rate of anti-KINECTIN antibody was higher in the group of patients with CD4+ decrease and CD8+ increase,than the ones with CD4+ normal or increase and CD8+ decrease or normal. Conclusion:Anti-KINECTIN antibody may play a significant role in the pathogenesis of aplastic anemia; Anti-KINECTIN antibody was associated with the clinical typs,treatment response and prognosis. Anti-KINECTIN antibody titre in AA patients'serum may be used as one of the indicators of the disease activity of AA. There is disorders of T lymphocyte subset in aplastic anemia.
