Purposes:With widely used of the treatment of operation in the treatment of all kinds of diseases ,before the operation of anesthesia also became more and more be concerned for people. For safety ,we are used to all people in all linen, mainly of the tracheobronchial intubation all linen ;because the skin of trachea is susceptible to damage and destruction, so for all linen in trachea is often accompanied with serious trachea intubation all linen complications,these complications is due to the staff of technology and the use of chemical materials,and these are mechanical damage and exciting complications, patients often have the clear throat hurts. As the development of medicine and medical knowledge,the needs of patients to hospital for the operation becoming more and more comfort, because the pain of the complications of anesthesia after the operation are could not be endured by patients ,the situation has already become the main sources of the disputes between doctor and patient.Therefore, whether from the standpoint of improving the quality of the operation or alleviating the disputes between doctor and patient, Reasonable solution to trachea intubation all linen complications become the serious events of our medical workers face.To the reasons of complications, we should strengthen improving the technology of workers and to use good material, recently we almost come to study some supplementary drug became a hot point.This research mainly through compare to the difference of whether supply compound lidocaine hydrochloride plasmagel to tracheal mucous membrane cell damage of trachea cannula complete hemp dog,to probe whether compound lidocaine hydrochloride plasmagel to tracheal mucous membrane cell have protective effect, for guide to relieve tracheal intubation complications of clinical will provide some help and theoretical basis.Methods:Choose 24 health hybrid adult dogs in 12-18 months old and there have no limit on female and male,the height is 15-25kg.Divide 24 health hybrid adult dogs into 3 groups at random,every group have 8 health hybrid adult dogs.The fist group:I group(contol group)there have no special handing on practical catheter of trachea cannula,postanesthetic for 8 health hybrid adult dogs to give trachea cannula,and divide the 8 health hybrid adult dogs into 2 groups at random,one group end the experiment after 2h was called 2h group,the 4 of rest end the experiment after 4h was called 4h group,after draw material from cuff for endotracheal catfeter make the experimental dog death.After draw material fix the organization to the special plate at once, divesting the epithelium of tracheal mucous membrane carefully and will have the achieved the organization of epithelium mucous membrane near on the surface of cartilage coated filteer paper, to immerse electron microscope sample stationary liquid to fix to provide examine. The second group:different from the first group,Ⅱgroup(experimental group) before trachea cannula,ahead of practical catheter of trachea cannula well-distributed coated with sterile lidocaine hydrochloride to lubricate and choose amyl colloid randomly(its contain 2.5%,lidocaine 5ml), postanesthetic for 8 health hybrid adult dogs to give trachea cannula,and divide the 8 health hybrid adult dogs into 2 groups at random,one group end the experiment after 2h was called 2h group,the 4 of rest end the experiment after 4h was called 4h group,after draw material from cuff for endotracheal catfeter make the experimental dog death.After draw material fix the organization to the special plate at once, divesting the epithelium of tracheal mucous membrane carefully and will have the achieved the organization of epithelium mucous membrane near on the surface of cartilage coated filteer paper, to immerse electron microscope sample stationary liquid to fix to provide examine. All dogs suffer anesthesia and trachea cannula by the same worker,in the experiment the kind of drugs and the dosage forms in the induction of anesthesia and maintenance of anesthesia at equal pace,in the experiment to retain experimental dog spontaneously breathing MR-9000 multifunction test HR,arterial pressure of damage.The method of anesthesia as following:fix the dog to lie ont the back in theoperating-table, in the condition of germ free,through the right femoral vein of the dog to piercing remaining needle, after make the vein dredging collaterals to inject pentobarbital 5mg/kgt all linen guidance, through the left femoral aeteriopuncture connection with the damage artery blood pressure determination sensors,sticking of precordial close the heart monitor( dog, the chest hair was razor) all the experimental dogs in 15-20 time/minute rate of breathing,50- 80 time/minute rate of heart rate,60-110 mmHGg of blood pressure the physiology state haven't trachea cannula ,use the view of trachea cannula to successful in the first time,the number of catheter is ID9, in aneroid gasbag determined table to determine pressure and make the pressure of cuff to 25 mmHGg, In the experiment to intravenous injection pentobarbital 2mg/kg/h to maintain anesthetize fractionation,and make the change of blood pressure of experimental dog in the scopeof土20% before trachea cannula,the rate of breathing in the scope of 15-20time/minute,the rate of heart in the scope of 10% before trachea cannula,to ensure the deep of anesthesia at equal pace stably,after intubation to start count time ,to anesthesia in 2h,4h respectively. The sample of fixed by stationary liquid through dehydration,transparent,osmic acid fix,epoxide resin embedding slice,and make to the need sample ,to observe the change of the subcellular structure morphology in the electron microscope ,and to evaluate the different index of every group.Results:Compared to control grouo ,the damage of experimental group cell structure is light,the degree of mitochondrial swelling is low;the cell nuclear ,nuclear membrane structure is clear ,in the experiment group the proportion of intranuclear heterchromatin is lower than control group obviously, the proportion of intranuclear have vacuole observational is lower than control group obviously,and compared to the sample ofⅠgroup and the 2h control empty sample group of no trachea cannula ofⅢgroup,can see experimental group have the prophase surface of damage cell,according to the mention of above text compare the four aspect index,the degree of experimental group sample cell damage is higher than control group,across compare to the experimental group and control group ofⅠgroup sample andⅡgroup sample to discover the damage of experimental group sample cell ofⅡgroup is lower than the control group sample cellofⅠgroup.According to the above observation,after compare to the damage degree ofⅠ,Ⅱ,Ⅲgroup sample cell,the marshaling sequence from light to height of the damage degree of every group sample cell isⅢ(2h)<Ⅲ(4h)<Ⅱ(2h) <Ⅱ(4h)<Ⅰ(2h)<Ⅰ(4h)Conclusion:Compared to have no coated with compound lidocaine hydrochloride plasmagel,the animals of coated with compound lidocaine hydrochloride plasmagel, compound lidocaine hydrochloride plasmagel have better protective effects to the the damage of mucous membrane epithelium cell structure of trachea cannula,especially in short time ,but with the prolong of catheter retain time in the animal, the damage of mucous membrane epithelium cell structure became more and more serious.
