ObjectiveTo analyze correlation of 1,5-anhydroglucitol(1,5-AG), short-term glycemic control, glucose fluctuations and oxidative stress activation.Methods1. From March to July in 2010, according to ADA 2003 diagnostic criteria,281 individuals with normal glucose regulation and other metabolism were recruited from Beijing community or outpatients of Chinese PLA General Hospital. Serum 1,5-AG levels were meagured with GlycoMark.2,Thirty-eight outpatients with type 2 diabetes from Chinese PLA General Hospital randomly received Lispro 25 interventions for 12 weeks. Patients regularly monitored their blood glucose levels before and 2h after main meals in order to obtain mean blood glucose(MBG), and 1,5-AG, glycosylated hemoglobin (HbAIC)and serum glycated albumin(GA) were measured at week 0,2,4,8 and 12 with clinic visits. Changes of clinical markers of glycemia before and after intervention were compared and analyzed by Pearson correlation coefficient.3,Seventy-one outpatients with type 2 diabetes were recruited from our hospital randomly. And their blood glucose levels were monitored by CGMS for three consecutive days. From 48h to 72h during CGMS period,24h urine samples were collected.Free 8-iso Prostaglandin F2α(8-isoPGF2α),8-hydroxy-guanine (8-OhdG) and serum malondialdehyde(MDA) were measured to evaluate oxidative stress. Fasting venous blood samples were collected to measure serum 1,5-AG, whole-blood HbAIC and serum GA. Clinical markers of glycemia, parameters of glucose excursion from CGMS, oxidative stress and other metabolic readouts were analyzed by Pearson correlation coefficient and multivariate stepwise regression.Results1,The reference value of serum 1,5-AG in normal population:1) The normal range of serum 1,5-AG for normal population could be suggested at 68μmol/L-251μmol/L overall,80μmol/L-267μmol/L for man and 66μmol/L-206μmol/L for woman.2) The level of 1,5-AG in man was higher than that in woman, but there was no difference in the whole range of age and BMI in subjects.2,Relationship between 1,5-AG and short-term glycemic control:1) Pearson correlation analysis:there were significant correlation between 1,5-AG, HbAIC, GA and MBG.1,5-AG was most closely associated with MBG within 2 weeks, while the correlation between HbAIC and MBG maintained longer than others.2) Comparison of longitudinal changes among clinical markers of glycemia:mean and median absolute percent changes versus baseline were markedly higher for 1,5-AG than for HbAIC and GA. After treatment of 2 weeks,1,5-AG values had already increased by 18.79%, but HbAIC and GA values decreased by 16.13% and 6.24%. By week 8,1,5-AG median values had increased by 65.18% versus diminution of values of 16.13% and 21.13% for the establishied markers of glycemia.3) In general, there was satisfactory concordance between longitudinal changes of 1,5-AG, HbAIC, GA and MBG in individual patient.3,Correlation of 1,5-AG and glucose excursions:1) Pearson correlation analysis: 1,5-AG was significantly correlated with MAGE, SDBG, M-value, MODD, PPGE and IAUC(r value was-0.509,-0.430,-0.462,-0.530,-0.223,-0.416 and-0.435, respectively, P<0.01), especially in moderately to well controlled patients by HbAic. Furthermore, statistical significance was also found in the correlation between HbAIC, GA and all parameters of glucose excusion. Negative correlation by multivariate stepwise regression analysis was found only between 1,5-AG and those parameters, but not HbAIC and GA in moderately to well controlled patients by HbAIC.2)Furthermore, HbAIC and GA were primarily positively correlated with MBG and FBG.4,Correlation of 1,5-AG and oxidative stress activation:1) With Lispro 25 interventions in T2DM patients,1,5-AG value increased 40.61%, while urinary excretion rates of 8-isoPGF and 8-isoPGF, MAGE and MODD readouts decreased by 20.24%,19.01% and 18.33% respectively(all P
