| ObjectivePo1ycystic ovarian syndrome(PCOS) whose main features are androgen excess and the chronic anovulation, is one of the most common endocrine disorder diseases for the women of childbearing age.The incidency rate accounts for 5%-21%for the infertility women and 75%-80%among the anovulatory women. In 1935, Stein and Leventhal first reported a group of syndrome principally characterized as amnenorrhea, obesity, hirsutism, anovulation and bilateral ovarian po1ycystic changes.Because of its very extensive changes of physiopathology which including in genetics, neuroendocrinology,g1ycometabolism,adipic-metabolism,protein-metabolism and abnormality of partial regulating and contro1ling factors of ovary, the research on PCOS still remains as one of the hottest topics in thefield of Gynecological endocrinology.Pcos mainly characterizes androgen excess along with chronic anovulatory.Hypersecretion of androgen in ovary is the key reason for the PCOS.It is crucial to understand the androgen excess-the characteristics changes in ovary,for elucidating polycystic ovary syndrome pathogenesis.In the literature,it has been suggested that PCOS is 1ikely to be a result of the cytokines regulator disorder。It is known that androgens mediatetheir action via the androgen receptor (AR), but the relation between the androgen receptor (AR) and PCOS is still not clear.For polycystic ovarian syndrome (PCOS) common polycysticovarysyndrome, Because of the endocrine features of high androgen of PCOS,AR of the PCOS patients'ovarian tissue is chosen to be studied in this thesis. The aim is to relate AR with the pathogenesis of PCOS and its clinical features.It can provide some theoretical guide to the prevention and treatment of PCOS.Methods:We took 15 cases of ovarian wedge resection of ovarian pathology due to PCOS, and 13 cases of normal (non-PCOS) ovarian tissue from age-matched patients at early stage of the menstrual cycle as control group. Ovarian follicles of two groups were counted by morphometric analysis. The expression of AR was analysed with immunohistochemical staining.Results:1.Stockpiling of growing follicles in ovaries of women with PCOS Two pathology physicians observed the pathological structure in the optical microscope double-blindly. 10 visual fields (10*10) were randomly selected, and the number of follicles at all levels was recorded. The total number of growing follicles is significantly greater in PCOS ovaries than normal, but the number of nongrowing primordial follicles is almost the same. The transitional primary follicles, typical primary follicles, secondary follicles and antral follicle are more than control group,and the differences are statistically significant (P<0.05). Differential counts showed that the number of growing follicles at each stage of development was significantly greater.2. Expression of AR in folliclesAR mainly expressed in the oocyte of primordial follicle and the oocyte,granulosa cells and theca cells of of primary follicles,secondary follicles,antral follicles and aretic follicles showing brown yellow. There are stronger intensities immunostaining in the PCOS group .A little higher AR was associated with immunostaining in the PCOS group(P>0.05), but the two groups AR immunohistochemical stains result comparison was not statistically significant.Conclusion:In ovarian tissue sections of PCOS patients, we found increased number of early growth follicles and antral follicles, AR mainly expressed in the oocyte of primordial follicle and the oocyte,granulosa cells and theca cells of of primary follicles,secondary follicles,antral follicles and aretic follicles showing brown yellow.There are stronger intensities immunostaining in the PCOS group .A little higher ARwas associated with immunostaining in the PCOS group(P>0.05). but the two groups AR immunohistochemical stains result comparison was not statistically significant. Which suggests that ovarian local groups of AR expression and PCOS a little positive correlation. |
