Objective: Through stakeholder analysis and related theory, this paper try to define the stakeholders of National Essential Medicine System, giving a comprehensive analysis to each stakeholder's character, powerinterest appeals, measures, also the power and resistance during the implementation of essential medicine system. The discussion has been focused on the impact on primary health care institutions and patients. Point out problems appearing during the implementation of the essential medicine policy; provide a basis for improving policies.Methods: The literature research, expert consultation, site survey, stakeholder analysis. Results: (1) stakeholders of essential medicine system including: government ministries (health administration department, the social security sector, pharmaceutical sector financial sector, Development and Reform Commission); medical and health institutions (especially primary health care institutions); pharmaceuticals production and circulation enterprises; doctors; patients. (2) the interests demands of all stakeholders, gains and losses of interest determine their behavior and attitudes, he main power sources of essential medicine system are government, large-scale pharmaceutical production, distribution companies, the health insurance sector, patients; major source of resistance; Financial funds, pharmaceutical production and circulation of small and medium enterprises, public hospitals, doctors. (3)the impact of essential medicine system on medical institutions is different, some institutions'service volume, income and positivity of medical workers has declined; overall price level of essential drugs dropped significantly the imperfection of essential drugs list and other factors affecting the drug using by patients .Conclusion Expend the essential medicine list take measures to ensure the production and supply of essential medicine new mechanism for centralized procurement of essential drugs should persevere and continue to be improved comprehensive reform of primary medical institution and county medical institutions should be promoted simultaneously with the essential medicine system improve the government's compensation policy "the fixed lump sum" should be promoted strengthen the publicity of essential medicine system improve the supervisory system use modern information and network technology for medicine supervision.
