Objective Hepatitis B recurrence is one of the most important reasons that afflect Long-term survival and quality of life of patients transplanted for HBV-related disease. the exact mechanism is unknown. By detection of HBV cccDNA and HBV DNA in PBMCs and liver tissue and serum from hepatitis B virus-related end-stage liver disease patients at different stages,we may be find a peripheral cellular location and the maybe way of hepatitis B recurrence after liver transplantation.Methods Isolate PBMCs from Blood by Lymphocyte separation medium. Detect HBVcccDNA in PBMCs and liver tissue and the serum HBV DNA in 37 cases of hepatitis B virus-related end-stage liver disease patients at three periods time of before and after liver transplantation and postoperative recurrence by Fluorescent quantitative PCR.Results In 37 cases of hepatitis B related end-stage liver disease before liver transplantation,HBV cccDNA in PBMCs and liver tissues were positive in 12 cases and 6 cases, the detection rates were 32.43% and 16.22%,HBV cccDNA copy in PBMCs range from 3.028 to 6.508× 104copies / ml, as in liver tissues the range was from 4.158 to 6.234×104 copies / ml. After liver transplantation, HBV cccDNA in PBMCs was positive in one case. In 6 cases of hepatitis B recurrence after liver transplantation,HBV cccDNA in PBMCs was positive in 4 cases,as positive in one case of Liver biopsy,and all 6 cases positive for serum HBVDNA.Conclusion After liver transplantation for patients with HBV-related liver disease, viral load has been cleared most of the case ,but remained virus in the PBMCs in accordance with cccDNA as a template re-replicate in the body, and then migrate to the liver, leading to Hepatitis B recurrence after surgery,and providing a new way of thinking for further prevention and treatment of hepatitis B recurrence after liver transplantation.
