Objective: to investigate the characteristics and clinical significance of para-recurrent laryngeal nerve lymphnode metastases in thoracic esophageal squamous cell carcinoma and to find out a reasonable extent for regional lymphadenectomy. Methods: The clinicopathologic and follow-up data of 59 patients with esophageal squamous cell carcinoma, underwent surgical treatment in thoracic surgery department of Cancer hospital of Shantou university medical college from May 1,2010 to Jan 31,2011 were analyzed retrospectively by univariateχ~ 2 analysis and multivariable logistic regression analysis. Results: Para-recurrent laryngeal nerve lymph nodes metastases were found in 11 of 59 patients(18.64%), with the left side metastases rate 5.08%(3/59) and the right side metastases rate 15.25%(9/59). The univariateχ~ 2 analysis showed that the differentiation grade of tumor, the metastatic status of thoracic lymph nodes, abdominal lymph nodes, total lymph nodes, subcarinal lymph nodes, para-esophagus lymph nodes, peri- left grastic artery lymph nodes were significantly associated with para-recurrent laryngeal nerve lymph nodes metastases (P<0.05). Multivariable logistic regression analysis showed that only the metastatic status of thoracic lymph nodes was statistically significant (P<0.05). Conclusions: Recurrent laryngeal nerve lymphnode metastasis of esophageal carcinoma has a high transfer rate. The recurrent laryngeal nerve lymphnode dissection can improve the thoroughness of esophageal carcinoma surgery and provide more accurate tumor staging, but also may reduce the relapse rate and improve survival.
