| Objective To investigate the effects and mechanism of hypertonic sodium chloride hydroxyethyl starch 40 injection (HSH40) on endotoxin-induced acute lung injury in rats. Methods Fifty healthy male SD rats weighing 250 g~280 g were randomly divided into 5 groups with ten animals each:groupC,groupL,groupH1,groupH2 and groupH3.The rats were anesthetized with 3% sodium pentobarbital 30~50 mg?kg-1.Lipopolysaccharide(LPS)5mg·kg-1 was injected to induce ALI through right femoral vein in group H1-H3 and groupL.1min after LPS IV, HSH40, HES, HTS and 0.9%NS were continuously intravenous infused through right femoral vein at a dose of 5ml·kg-1 respectively; groupC served as control one by 0.9%NS of the same volume.After 4 hours, blood samples were taken from right femoral artery.Then the rats were sacrificed and the lung tissue samples were collected . PH,PaO2,PaCO2 and [K+],[Na+],[Ll-] were analyzed.The wet/dry (W/D) lung weight ratio,the albumin concentration of bronchoalveolar lavage fluid (BALF) and activity of myeloperoxiase (MPO) were detected. The levels of TNF-αin serum were assayed by enzyme immunity. Pathologic changes of lung tissue were observed and graded microscopically. The expression of ICAM-1 in lung vascular endothelium was measured by immunohistochemical.Results①PaO2 were significantly lower and PaCO2 was significantly higher in groupL than those in any other group (P<0.01). PaO2 were obviously higher and PaCO2 was obviously lower in groupH1 than those in groupH2,H3 (P<0.01).②The concentrations of Na+ and Cl- were significantly higher in groupH1,H3,and this rising was more remarkable in groupH3 (P<0.01) .At the same time,the concentrations of K+ was significantly lower (P<0.01) in groupH3.③The W/D and the albumin concentration in BALF were significantly lower in groupH1,H2 and H3 than those in groupL (P<0.01),but this decreasa was more remarkable in groupH1,H2 (P<0.01).There was no different in that of groupC and H1 (P>0.05).④The levels of TNF-αin serum and MPO activity in the lung were significantly higher in groupL than those in groupH1,H2 and H3 (P<0.01) .Compared with H1,these two indexes were increased in H2 (P<0.05) and were significantly increased in H3 (P<0.01) .⑤Pathological damages of lung in groupH1-H3 were severely less than that in groupL.Especially groupH1 had the most remarkable effects to inhibited damages in ALI.But the pathologic grade was no different in that of groupH1-H3 (P>0.05) .⑥The expression of ICAM-1 in lung vascular endothelium was obviously lower in groupH1-H3 than that in groupL (P<0.01) .Compared with H1,ICAM-1 was increased in H2 (P<0.05) and were significantly increased in H3 (P<0.01) .Conclusion HSH40 has protective effect on ALI induced by endotoxin and has significantly advantage than HES130/0.4 and HTS. |
