Objective:Using real-time fluorescent quantitative RT-PCR method, combine detection tumor markers CK20mRNA and COX-2 mRNA, Detection the cancer cells in peritoneal lavage fluid of gastric cancer patients, Study the value of combined detection CK20mRNA and COX-2 mRNA in predictive peritoneal metastasis of gastric cancer.Methods:Peritoneal washes were collected from 36 patients with gastric carcinoma during a laparotomy. CK20mRNA,COX-2 mRNA levels in the peritoneal washes were detected by real-time fluorescence quantitative reverse transcription polymerase chain reaction. Another peritoneal washes were collected from 16 patients with benign.8 gastric cancer tissue samples as the a negative control.Results:1.Among 36 cases of gastric carcinoma,in peritoneal washings,positive expressions of CK20mRNA,COX-2 mRNA were 61.1%(22/36),52.8%(19/36).The positive of combined detection CK20mRNA and COX-2mRNA were 72.7%(26/36),the positive were higher than the PLC detected 19.4%(7/36).2.7 patients with cellular pathology-positive were positive detected with CK20mRNA and COX-2 mRNA,29 patients with negative cytology test,15 cases were detected positive with CK20mRNA and 12 cases detected positive with COX-2 mRNA.3. None of 8 patients with benign disease were positive. In the 8 cases gastric carcinoma tissue, the detection of CK20mRNA,COX-2 mRNA were positive. And the quantitative value is much higher than peritoneal lavage fluid.Conclusion:1.The detection of exfoliated cancer cells in n peritoneal washes by real-time fluorescence quantitative reverse transcription polymerase chain reaction were more sensitive than PLC, And it traditional RT-RCR can reduce the false-positive rate detected by traditional RT-RCR and false-negative rate of PLC; 2. CK20mRNA and COX-2 mRNA has a good specificity, can be used as a more valuable tumor marker; 3. Combined detection CK20 mRNA and COX-2 mRNA can increase detection the rate of gastric cancer peritoneal cytology; 4. COX-2 mRNA were the targets that reflects the invasive properties of tumor, combined with CK20 mRNA can be usede for screening the high-invasive cancer, For predicting peritoneal metastasis and prognosis of gastric cancer is more valuable than the detection of CK20 mRNA alone.
