The Fluence Of Trabscatheter Arterial Chemoembolization On The Feeding Arteries Of Primary Hepatic Carcinoma

Objective:Analisizing the fluence of Trabscatheter Arterial Chemoembolization on the Artery of Primary Hepatic Carcinoma.Methods:By retrospectively researching 128 patients of PHC of our hospital who have accepted more than twice TACE during Jan,2006-Jan,2010,to observe the relation between collateral circulation and the position of tumor,the times of cure,to observe the conditions of hepatic artery's stenosis and obstruction after many times therapys,to observe the effect of using different embolism methods to embolism blood-supply's vascular.Results:1.The number of collateral circulation are 59,among the total hepatic lateral branch are 42,hepatic medial branch are 17.Being the increasing of interventional therapy times,the incidence rate of hepatic lateral branch is raising,the incidence rate are respectively 28%(the second time),34.2%(the third time),58.8%(the quart time),72.7%(the fifth time and above).Using chisquare test to analysiz the relation between interventional therapy times and the incidence rate of hepatic lateral branch,the result is:As the increase times of therapy the incidence rate of hepatic lateral branch is raising.2.The number of the stegnosis and(or)obstruction of hepatic artery are 28 examples.the incidence rate are respectively 14.5%(the second time),21%(the third time),35.3%(the quart time),54.5%(the fifth time and above). Using chisquare test to analysiz the relation between interventional therapy times and the incidence rate of hepatic lateral branch,the result is:As the increase times of therapy.the incidence rate of the stegnosis and(or)obstruction is raising.3.We have three methods of embolism,we separate them from three groups,and analysizing the effect of different embolism methods on revascularization by using chisquare test.The result is no-recanalization rate of group method 2 and group method 3 is obviously more than that of group method 1,and part-recanalization and all-recanalization rate of method 2 and group method 3 obviously is less than that of group method 1.Conclusions:The emergence of collateral circulation and recanalization are precipitating cause of hepatoma's recurrence and metastasis after TACE. As the increase times of therapy,the incidence rate of hepatic lateral branch and the incidence rate of the stegnosis and(or)obstruction are raising.the cituation to deposit of the lipiodol is related with embolism methods.
