Objective:(1)To develope a scale for measuring nurses'clinic communication competency; (2)To examine the reliability and validity of the scale; (3)To investigate the situation of nurses'clinic communication competency.Methods:The item pool was obtained through reviewing literature and interviewing with senior nurses. Five experts were invited to assess the primary scale and it was modified according to experts'suggestions. Four hundred and twenty one nurses from grade 2 and 3 hospitals in the city of Changsha participated in the first study by cluster sampling and demography questionnaire, nurses'clinic communication competency scale (test edition) and General Self-Efficacy Scale (GSES) were uesed. The final scale was abtained after the first study. In the second study, one thousand and two hundred nurses from grade 2 and 3 hospitals in the city of Changsha were recruited and they finished the questionnaires including demography questionnaire, nurses'clinic communication competency scale(final edition) and General Self-Efficacy Scale(GSES). All data was input into SPSS 13.0. The intem analysis methods included item distribution, coefficient of variation (CV), t-test, Pearson correlation coefficients, factor analysis and Cronbach a. The structure validity was examined by exploratory and confirmatory factor analysis. The content validity was examined by the the scale's formation process. Cronbach a, split reliability and test-retest reliability were used to examine the reliability. Descriptive statistics, t test, F test, Pearson correlation analysis and multiple linear regression analysis were also used to analyze the data.Results:(1) Two items were deleted from the 60-item primary scale through item analysis. The final scale contained 6 dimensions:the team communication competency, bacsic language communication competency, basic non-language communication competency, emotional perception competency, emotional support competency and communication competency in difficult clinic scene. Exploratory analysis of the scale's items identified a six first-order factors model that accounted for 59.476%. In the confirmatory analysis,X2/ df was 2.890, RMSEA was 0.056, NFI was 0.798, RFI was 0.781, IFI was 0.858, TLI was 0.845, CFI was 0.857, PNFI was 0.737, PCFI was 0.791. The scale's content validity was good. The scale's cronbach's a was 0.978 and the dimensions' cronbach's a ranged from 0.868 to 0.954. The scale's split coefficient was 0.923 and the dimensions'ranged from 0.765 to 0.916. The scale's test-retest coefficient was 0.727. (3)The mean score of nurses'clinic communication competency was 3.93±0.49. The score of dimensions ranked by descending were:team communication competency, basic non-language communication competency, basic language communication competency, emotional perception competency, emotional support competency and communication competency in difficult clinic scene. Nurses'clinic communication competency score was significantly different from hospital grades, work time, the post title, positions, the primary degrees, communication training times, the extent of communication knowledge (P<0.05) and the general self efficacy was positively related with clinic communication competency (r=0.41, P<0.01). Stepwise regression analysis revealed that the general self efficacy(β=0.370), the extent of communication knowledge (β=0.233), communication training times(β=0.129), the post title (β=0.062) and hospital grades (β=-0.061) were included in the regression equation in which clinic communication competency was as dependent variables(F=67.899, P<0.01).Conclusion:The Nurses'clinic communication competency scale contains 6 dimensions:team communication competency, basic language communication competency, basic non-language communication competency, emotional perception competency, emotional support competency and communication comeptency in difficult clinic scene. The scale's validity and reliability is good. The related factors of nurses' clinic communication comptency contain hospital class, work time, the post title, position, the primary degree, communication training times, the extent of communication knowledge and the general self efficacy.
