Objective To analyze the prognostic impact and Risk Factors of solitary lymph node(LN) metastasis on Cervical Carcinoma. Methods 188 patients with diagnosis of stage IA-IIB(FIGO) cervical cancer were prospectively studied from January 1999 to December 2005, 188 patients with cervical cancer underwent radical hysterectomy and pelvic lympha -denectomy. Among them, 43 were proven solitary lymph node metastasis pathologically(solitary LN metastasis group),while 145 proven node negative metastasis (node-negative group). The 5-year survival rates were compared between solitary LN metastasis and node-negative groups. The prognostic factors were evaluated by univariate and multivariate analyses. The multivariate analyses was used to study the relationship of clinical and pathological features of cervical carcinoma with metastasis of pelvic lymph node. Results The 5-year survival rates of solitary LN metastasis group and node-negative group were 51.2% and 84.1% respectively, with significantly difference(P<0.05). The solitary LN metastasis was one of the independent prognostic factors for the patients enrolled. Logistic regression revealed that the depth of invasion,clinical stage and lymph vascu1ar involvement were independent covariates for solitary LN meta -stasis. Conclusions When patients had solitary LN metastasis, the prog -nosis of them was significantly worse than those without lymph node metastasis. The major risk factors of lymph nodes metastasis in cervical carcinoma are muscular infiltratio,lymph vascu1ar involvement and clinical stage.
