OBJECTIVE:Through the simulated surgery of head and neck specimens, explore the feasibility of endoscopic transoral surgery of expanding line of occipital bone, providing some experimental evidence for the clinic treatment of basilar invagination.METHODS:Select complete specimens of 20 head and neck, the specimens were randomly divided into two groups, the first groupâ… ,10 (7 male and 3 females), for the traditional group; Groupâ…¡10 (7 male and 3 female), for the endoscopic group. Respectively Transoral line simulation occipital augmentation surgery, Canon group were MED-â…¡type surgery system.RESULTS:1. Traditional group and the endoscopic group could complete the exposure and the partial excision of occipital bone and the slope.2. Both traditional group or Canon group, in order to reveal the posterior pharyngeal wall, the soft palate must be cut without the incision of hard palate.3. Traditional group required a 5cm long incision in the median posterior pharyngeal wall while endoscopic surgery group for only 4 cm in the same area.4. Bone window in the ramp width of 15mm, height 10mm, depth of 6mm, meet Transoral foramen magnum augmentation requirements and a certain security.5. Operations between the traditional and endoscopic method have no significant difference in removal of the slope, the width and height of foramen magnum decompression, showed that endoscopic surgery can realize the same effect in expansion achieved by traditional surgical.6. In terms of polishing the slope, endoscopic surgery is possibe to precisely control the polishing width, directly observe the depth of drilling, and less easy to damage dural.7. Retrogradation and grinding division can achieve partial excision of the slope, in order to avoid the damage of the rear dura, as selecting grinding divisionCONCLUSIONS:This study, through comparison between traditional and endoscopy methods in cadavers with transoral-line expansion of the foramen magnum surgery simulation operation, showed that endoscopic transoral-line expansion of the foramen magnum surgery is technically feasible, endoscopy surgery can not only reach the scope of traditional expansion surgery, but reveal more clearly the local structure and prove the operation more convenient and accurate,this surgery is superior to the traditional group.
