Objective:To investigate the effect of hyperbaric oxygen (HBO) and tanshinoneⅡA (TanⅡA) on the expression of hypoxia inducible factor-1alpha (HIF-1α) in renal tissues through a renal ischemia-reperfusion injury model in rats. Meantime, the mechanisms of renal ischemia-reperfusion injury were also explored, which would provide theoretical proofs for clinical application of HBO and TanⅡA on prevention of renal ischemia-reperfusion injury.Material and Methods:84 Wistar rats were divided into 5 groups by random:normal group, ischemia group, ischemia-reperfusion group (1h,3h and 5h), HBO group(1h, 3 h and 5 h), TanⅡA group (1h,3 h and 5 h) and sham operation group (1h,3h and 5 h),Each group had 6 rats. Firstly, the right kidneys of rats were removed. A piece of cortico-medulla was cut out to make paraffin block by 4% Polyoxymethylene to fix in every left renal, HE staining was performed and the expression of HIF-1αprotein was detected by immunohistochemistry through the paraffin section. The other moiety was stored by subambient temperature to extract the total RNA and follow to detect the expression of HIF-1αmRNA through real-time PCR. After resection the left renal get blood through pricking inferior vena cave to determine BUN, SCr, MDA and TNF-α.Results: ①It was statistical significance(P<0.05) compare BUN in sham operation 5h, the expression of HIF-1αprotein in sham operation 3h and HIF-1αmRNA in the sham operation 1h with the normal group.②The levels of BUN, SCr, MDA and TNF-α in serum increased significantly at the normal group, ischemia group and ischemia-reperfusion group 1h,3h and 5h in turn(P<0.05). It was high-expression about HIF-1αmRNA in ischemia group and ischemia-reperfusion 3h group, the low-expression in ischemia-reperfusion 1h,5h group. High-expression about HIF-1αprotein was found in ischemia group and ischemia-reperfusion 1h,3h and 5h group.③All of BUN, SCr, MDA, TNF-α, the expression of HIF-1αprotein were lower, however the expression of HIF-1αmRNA were rise in TanⅡA group than ischemia-reperfusion group(P<0.05).④All of BUN, SCr, MDA, TNF-αwere lower and the expression of HIF-1αmRNA were higher in HBO group than ischemia-reperfusion group(P<0.05), however the expression of HIF-1αprotein no difference in HBO group compare with ischemia-reperfusion group(P>0.05).⑤Correlation analysis:According to the results about the expression of HIF-1αmRNA and HIF-1αprotein in renal with MDA, TNF-α, BUN and SCr in serum to correlation analysis and discovered, it were positive correlation about MDA with BUN (r=0.495, P=0.000), MDA with SCr (r=0.244, P=0.048), TNF-αwith BUN(r=0.702, P=0.000), TNF-αwith SCr(r= 0.435, P=0.000), TNF-αwith MDA (r=0.305, P=0.043), TNF-a with HIF-1αprotein(r=0.257, P=0.037) and BUN with SCr(r=0.514, P=0.000), however, it were negative correlation about BUN with HIF-1αmRNA (r=-0.260, P=0.035).⑥HE staining:It was no much change in the sham compare with normal group, small vessels congestion in renal interstitium and renal glomerulus with ischemia group. The other groups were found different extent hyperemia in glomcrulus and renal interstitium, different extent cellularedema, hyalinization and fatty degeneration in renal tubular epithelial cell. It was obviously that Protein cast in renal tubules of the ischemia-reperfusion every group. But it was individual in HBO and TanⅡA every group and the cellularedema abatement, and all in all, it is more abatement in TanⅡA than HBO ones with these pathological changes.Conclusions:①Have a change about BUN and HIF-1αto the trauma of operation.②It were the important injury causes that the increase of MDA, TNF-αand the expresion of HIF-1αprotein after renal ischemia-reperfusion.③It was possible protective mechanism for TanⅡA to depress the expression of HIF-1αprotein after renal ischemia-reperfusion. And it was the same path to express oxygen free radica and TNF-αfor TanⅡA and HBO after renal ischemia-reperfusion.④HBO could not depress the expression of HIF-1αprotein maybe can see HBO did not through the path of HIF-1αto protect renal injury after renal ischemia-reperfusion.⑤It was maybe indicate that TanⅡA had two-ways regulation mechanism to the expression of HIF-1αin renal ischemia-reperfusion injury through both TanⅡA and HBO can increase the expression of HIF-1αmRNA, meanwhile, only the TanⅡA can depress the expression of HIF-1αprotein after renal ischemia-reperfusion.
