The Bone Metabolism Changes Of Diabetic Rats' Orthodontic Treatment Under The Control Of Insulin

Abstract/Summary:
Objective: Through the establishment of diabetic models of rats, check the concentration level of the ICTP and PICP in the rats'alveolar bone using the ELISA method. And can investigate the infection that under the control of insulin, the bone metabolism of the diabetes mellitus mates during the otm orthodontic tooth movement, to find the experimental evidence and the theoretical basis for the clinical orthodontic therapy of the diabetic.At present, through the determination of biochemical indicator can get the changes level of the process of metabolism, such as osteocalcin (BGP), bone-specific alkaline phosphatase (BAP) and urinary pyridinoline crosslinks pyridinoline (Pyr), deoxypyridinoline (D2Pyr),â… procollagen carboxy-terminal propeptide (PICP) and typeâ… collagen cross-linked carboxy-terminal peptide (ICTP). PICP which reflects the bone cell activity is a sensitive and specific indicators, while ICTP is sensitive indicators that reflect the osteoclastic bone collagen degradation.Methods: 1. Choose one hundred and five healthy male SD rats whose weight is range from 200 to 220 grams and put the rats into 3 groups randomly ,each group includes 35 rats. Group A: the normol group; group B: diabetes group and group C: insulin treated group. Each group is randomly devided into five little groups each one has 7 rats, which will be cured for 0 day, 7 day, 14 day, 21 day and 28 day. Inject streptozotocin (STZ) into the rats'abdominal of Group B and Group C with the amount of 50mg per kilogram. Rats in Group A are injected with sodium citrate with the same amount. Seventy-two hours later, extract the blood from caudal vein and test the glucose in urine. The rates whose blood glucose is above 16.8mmol/L and urine glucose is between three plus and four plus are diagnosed as diabetes. Four days later, after the model of diabetes is founded, inject insulin under the skin of necks of the rats in Group C by 0-4U per day. Observe the blood glucose prudently, and then, modify the amount of the insulin, ensure the stochastic blood glucose is between 8 to 10 mmol/L. Group A and B are injected with the same amount of saline. Three weeks later, under total paralysis, fix the fixture in their right maxillary incisors and first molars, and then give the fixture a force of 30g. During the whole treatment, all rats are free to eat and drink. The little group of 7 rats are killed by abstracting blood from their hearts on 0, 7, 14, 21 and 28 days.2. Blood glucose determination: Blood glucose had been measured before rats were killed. Which rat's blood glucose in Group B is above 16.8 mmol/L, and less than 10 mmol/L in Group C is chosen to be examined for results.3. Collection of specimens: execute the tats with exsanguinating from the heart, and then pull out the first and the second molar, cutoff the alveolar bone of the medial sides of the second molar at two-thirds of the alveolar bone. Cutdown the alveolar around the first molar, weighing it with electronic balance, and then put it into the refrigerator which is 20 degrees below zero centigrade until it is tested.4. Test: test the concentration of the PICP and ICTP by Enzyme Linked Immunoadsorbent Assay.Results: 1. Changes of the PICP's concentration In group A and C, the concentration of the groups of 7and14 days are lower than that of 0 day ,but it is meaningless and then it up to the highest at 21 days ,then ,returned to the first as 0 day (P>0.05). In group B, the concentration level at 7 is lower than 0 day (P<0.01), and the 14th day is the lowest (P<0.05), and then it up to the highest at 21 days (P<0.01), at last, it is the same as 0 day at 28 days (P>0.05).It's meaningless that the concentration of group B is lower than group A at 7 days (P>0.05), and the others are much lower than group A and C (P<0.01).The concentration of group C had nothing different with group A(P>0.05), except 7 days(0.010.05). But the concentration of group B at 7 days and 14 days are higher than group A and C(P<0.05).Conclusion: 1. Under the treatment with insulin, the orthodontic tooth movement of diabetic rats could be attend, but during the tooth movement, the blood glucose should be strictly controlled and their clinic symptoms should be also carefully observed.2. Those diabetic rats without treatment with insulin show a tendency of osteoporosia, Therefore, orthodontic tooth movement should be carefully chosen.
