The contraction and diastole of the detrusor urinae control the storage and discharge of urine through influencing the bladder pressure and volume. The electro-acupuncture(EA) therapy treat the urination disorder by modulating the function of detrusor urinae of bladder.Objective:Elucidating the modulating effect of EA on the function of detrusor urinae of bladder, to provide the theoretical foundation for the selection of acupoins and parameters of electroacupuncture therapy for the urination disorder.Method:Male New Zealand Rabbits were used to conduct the experiment. Filling phase and urination phase of the bladder were produced artificially. Use the urindynamic method to detect the filling phase detrusor pressure wave(P), initial flow rate of the urination phase(V), flow rate in every end of a minute(Vt), volume of residual(L) and the ratio of the unstable type of the wave(R)(the detrusor pressure wave was divided into stable type and unstable type according to shape of the wave). V,Vt,L were regarded as the index of model establishing and the effects of EA.1. Grouping:Male New Zealand Rabbit were divided into 3 groups:control group(CG,n=20), model group(MG,n=20) and EA group(EA,n=60). The EA group was divided into 6 sup-groups according to the different acupoins and electro-parameter:2Hz Zhongji group (RN3,n=10),100Hz Zhongji group (RN3,n=10),2Hz Sanyinjiao group (SP6,n=10),100Hz Sanyinjiao group (SP6,n=10),2Hz Quchi group (LI3,n=10),100Hz Quchi group (LI3,n=10).2. Exploring the method of establishing the model:exploring the rate and the time of the artifial filling of the bladder and choosing the appropiate concentration of racanisodamine hydrochioride(654-2) to establish a detrusor urinae function weakened model with R decreased, V slowdown and L increased.3. R,V,Vt and L were detected as the the index for the detrusor urinae funtion. The effect of EA on the the detrusor urinae funtion were observed and the differences of each two EA groups were compared.4. Part of the results are presented as the Mean±SD. Correlation analysis between Vt and t. All analyses were performed using the SPSS statistical program (version 13.0).Results:1. The establishment of the detrusor urinae function weakened model.We observed the influence of 0.3g/L and 0.5g/L 654-2 (20ml/h, i.v) on the bladder urodynamic with 1.5h of artificially filling. The results showed that 0.5g/L 654-2 significantly decreased R(P<0.05), slowdowned V(P<0.05) and increased L(P<0.05), indicating that the higher concentration of 654-2(0.5g/L) could successfully establish a detrusor urinae function weakened model.2. The modulation effects of the different frequency EA in the differente acupoint to the detrusor urinae function weakened model.2.1 Effects on the wave shape of the filling phase detrusor pressureEach R from the EA group was higher then the model group, but the difference was not significant(P>0.05), indicating that the different frequency EA in the differente acupoint has no significant influence on the filling phase detrusor pressure.2.2 Effects on the Vt in the urination phaseCompared with the model group,2Hz EA(2Hz Zhongji group,2Hz Sanyinjiao group,2Hz Quchi group) accelerated V(P<0.05), while in the 100Hz groups, only 100Hz EA Sanyinjiao accelerated V (P<0.05).Analysing correlation between Vt and t, we got a imitative straight line equation Vt=b—Kt (b stands for the intercept, representing; K stands for the slope, representing the decending speed of the flow rate in the urination phase). We compared the b and K from each group. The results are as follows:each b from the EA group was bigger than the model group (P<0.05), indicating that the different frequency EA in the differente acupoint has significant modulation effects in the urination phase, significantly accelerate the initial flow rate of the urination phase. Each k from the 2Hz EA group was bigger than the model group (P<0.05). While each k from the 100Hz EA group has no difference with the model group (P>0.05). The results indicate that 2Hz may significantly accelerate the decending speed of the flow rate in the urination phase, while 100Hz EA has no influence on it.Multiple comparison among the groups with 2Hz EA showed that the b in the 2Hz Sanyinjiao group has no difference with the 2Hz Zhongji group(P>0.05); but they are bigger than the b in the 2Hz Quchi group (P<0.05). The results indicated that the modulation effects of 2Hz EA was acupoins specific, the effects of different acupoints range in the following order:Sanyinjiao and Zhongj,Quchi.Multiple comparison among the groups with 2Hz EA showed that there is no difference among different acupoints group in b, indicating that the modulation effects of 100Hz EA was acupoins specific.2.3 Effects on the volume of residual(L).The L in the 2Hz Zhongji group, 100Hz Zhongji group,2Hz Quchi group and 100Hz Quchi group were less than the model group, indicating that EA in the Zhongji and Quchi may significantly decrease the volume of residual of bladder.Conclusion:1.654-2(0.5g/L, i.v.,20ml/h) could successfully establish a detrusor urinae function weakened model with the characteristics of R decreased, V slowdown and L increased.2.The modulation effect of EA on the weakened detrusor urinae function is different in the filling phase and the urination phase. On the filling phase the effect of EA is slim, while on the urination phase detrusor, the effect is significant which menifested as EA signifiantely accelerate the initial flow rate of the urination phase.3.2Hz EA is superior to 100Hz EA in accelerating the initial flow rate of the urination phase.4.The effect of 2Hz EA in accelerating the initial flow rate of the urination phase is acupoints specific. The acupoints in the lower limb and the lower abdomen (Sanyinjiao and Zhongji) is superior to the acupoint in the upper limb (Quchi)5.The effect of 100Hz EA in accelerating the initial flow rate of the urination phase is not acupoints specific.
