Objective:To observe the imaging features of CT perfusion in hepatic alveolar echinococcosis (HAE) and analyze the correlation between CT perfusion parameters of peripheral zone of HAE and microvessel density (MVD), and to investigate the clinical value of CT perfusion technology in evaluating the hemodynamic changes of HAE. Methods:28 cases with HAE underwent GE 64-slice spiral CT perfusion. The perfusion parameters of the center, the peripheral zone and the around liver tissue of lesion were measured through liver perfusion analysis software, which including blood flow (BF), blood volume (BV), mean transit time (MTT), permeability surface area product (PS), hepatic arterial fraction (HAF), hepatic arterial perfusion (HAP), portal vein perfusion (PVP),respectively. Of the 28 cases with HAE, 17 cases underwent operations. After evaluation of the fresh lesions and the slides were stained with routine HE. MVD of peripheral zone of was examined by immunohistochemical techniques. The correlation between CT perfusion parameters and MVD were analyzed. Results:There was statistically significant difference in BF, BV, MTT, PS, HAF, HAP and PVP among the groups of the center, the peripheral zone and the around liver tissue of 28 patients with 31 lesions. BF, MTT, PS, HAF, HAP and PVP between the groups of the peripheral zone of lesion and the around liver tissue of lesion were statistically significant ( P < 0.05). However, the BV and HAF between the two groups were not statistically significant ( P > 0. 05). Positive relations were shown between the BF, HAF, HAP of peripheral zone of lesion and MVD. Conclusion:The blood supply of peripheral zone of HAE is much more than that in the center of the lesion and around liver tissue. CT perfusion technique is of clinical significance in analyzing the change of the blood flow of peripheral zone and invasive extent of HAE.
