Effect Of Circumferential Pulmonary Vein Ablation For Paroxysmal Atrial Fibrillation On Left Atrial Function

Objective:To evaluate the effects of circumferential pulmonary vein ablation(CPVA)with the application of echocardiography for paroxysmal atrial fibrillation(PAF)in patients on left atrial(LA) function. Methods:Eighty one PAF patients who underwent CPVA,and all the patients were didived into two groups according to the recurrence of AF ; before ablation,echocardiography parameters including LA active emptying volume(LAAEV),LA active emptying fraction(LAAEF),LA maximum volume(LAVmax),LA total emptying volume(LATEV),LA total emptying fraction (LATEF),A peak velocity time intergral(A-VTI) and LA ejection force(LAEF) were measured;three days,three months and twelve months after ablation,those parameters were measured again. Results:Group of unrecurrence,Compared with preablation ,the changes of LA pump function such as LAAEV[(7.6±2.8)cm3 VS (5.5±3.1)cm3],VA[(62.2±12.3)cm/s VS(52.5±10.5)cm/s],A-VTI([4.4±1.2)cm VS (3.5±1.2)cm] and LAEF([15.5±3.5)kdaynes VS (13.5±4.0)kdaynes]decreased significantly three months after ablation (P<0.05);LAAEV[(7.6±2.8)cm3 VS (8.1±2.9)cm3],VA[(62.2±12.3)cm/s VS (64.3±11.2)cm/s],A-VTI[(4.4±1.2)cm VS (4.8±1.4)cm] and LAEF[(15.5±3.5)kdaynes VS (16.4±3.9 ) kdaynes] increased significantly twelve months after ablation(P<0.05),and the indicator of LA reservoir function such as LAVmax[(34.4±5.2)cm3 VS (36.4±4.3 ) cm3],LATEV[ ( 14.5±3.3 ) cm3 VS (16.5±3.7 ) cm3] and LATEF[(58±21)% VS (59±20)%] increased significantly twelve months after ablation(P<0.05).group of unrecurrence,Compared with preablation,LAAEV[(7.4±2.5)cm3VS(5.1±2.9)cm3],VA[(60.2±11.3)cm/s VS (50.0±9.2)cm/s],A-VTI[(4.3±1.2)cm VS (3.2±1.3)cm] and LAEF[(14.5±3.3)kdaynes VS (11.4±3.7)kdaynes] decreased significantly twelve months after ablation(P<0.05). Conclusion :CPVA have negative impact on the LA function in the near future ,it have good impact at a specified future date.
