Background and objectiveGastric Cardia Carcinoma(GCC) is a high incidence of malignant tumors in Chaoshan, the incidence of GCC was 34.81/10,000 , second only to esophageal cancer (EC) 74.47/10,000 , esophageal (gastric cardia) cancer mortality rate about 105 /100 000. The pathogenesis of GCC is not clear, hidden, poorly differentiated, invasive strong infiltration of a wide range of poor prognosis. Therefore, to determine the cause of GCC effectively prevent GCC is significance.Helicobacter pylori infection was confirmed as the cause of distal gastric cancer, but the relationship between Helicobacter pylori infection and GCC is controversial. Nearly two decades, the prevalence of H. pylori infection in developed countries decreased, the incidence of distal gastric cancer decreased too, but there was an upward trend in the incidence of GCC. United States and other Western countries and Japan reported there was a negative correlation between Helicobacter pylori infection and gastric cardia /esophageal disease, some scholars believe that H. pylori infection may be a protective factor of esophageal and cardia disease. The studies based on the general investigation population of Taihang Mountain area in China and Iran showed high incidence of GCC is related to Helicobacter pylori infection. The incidence GCC and H. pylori infection need more research data to confirm that.We aimed to investigate the situation of Helicobacter pylori infection in patient suffered gastric cardia carcinoma by Giemsa stain and immunohistochemistry stain.Observe the role of Helicobacter pylori infection in mucosal inflammation and hyperplasia.Materials and methodsWe investigated the infection status of H. pylori and determine the grade of clump density of H.pylori by the Giemsa stain and immunohistochemistry in formalin-fixed, paraffin-embedded tissues sections from 372 patients with histological diagnoses of gastric cardia carcinoma. The expression level of Ki-67 was assessed by immunohistochemistry and then quantified by calculating a labeling index (LI).In the end, we analysis the data in appropriate statistical method.Result1. The prevalence of H. pylori infection of patients with gastric cardia carcinoma in Chaoshan was 81.5%, higher than the prevalence of general investigation population in Chaoshan (67%), higher than the overall prevalence in China (59%).2. The prevalence of Helicobacter pylori infection of mucosa adjacent to cancer (82.3%) and mucosa of distant cutting margin (80.3%) higher than the prevalence of carcinoma(67.2%) (P<0.01), Helicobacter pylori prevalence between adjacent mucosa and the margin mucosa was no significant difference (P> 0.05).3. It was observed in this study there was varying degrees of inflammatory cell infiltration and proliferation with Helicobacter pylori infection. Inflammation grade of mucosa adjacent to cancer was higher than of gastic cardia of general investigation population in Chaoshan (P<0.01). Inflammation grade correlated with the hyperplasia grade of mucosa adjacent to cancer (rs =0.36,P<0.01). Inflammation grade correlated with the hyperplasia grade of mucosa adjacent to cancer (rs =0.36,P<0.01). Expression of Ki-67 increased with inflammation grade in mucosa adjacent to cancer and mucosa of distant cutting margin (P<0.05).Conclusion1. The prevalence of H. pylori infection of patients with gastric cardia carcinoma in Chaoshan was 81.5%, higher than the prevalence of general investigation population in Chaoshan (67%), higher than the overall prevalence in China (59%).2. The prevalence of Helicobacter pylori infection of mucosa adjacent to cancer and mucosa of distant cutting margin higher than the prevalence of carcinoma. 3. There was varying degrees of inflammatory cell infiltration and proliferation with Helicobacter pylori infection in mucosa adjacent to cancer and mucosa of distant cutting margin. Chronic inflammation correlated with the hyperplasia, hint Helicobacter pylori infection may be correlated with occurrence of gastric cardia carcinoma.
