Objective: To compare the clinical effect on the treatment of unstable intertrochanteric fracture in the elderly between the DHS fixation and the artificial femoral head prosthetic replacement, and to approach its indication and methods.Methods: Totally 50 intertrochanteric fractures in senile patients were retrospectively analyzed,50 old patients (31 woman and 19 man) suffered from the Osteoporosis unstable intertrochanteric fracture according to Evans typing system were selected from January 2004 to December 2007,They were combined with severe osteoporosis, 15 cases average age was 76.5 had been treated with the artificial femoral head prosthetic replacement, The average duration of follow-up for the artificial femoral head prosthetic replacement was 26 month,35 cases average age was 72.1 had been treated with DHS internal fixation, The average duration of follow-up for the DHS internal fixation was 23 months, Analysis on the following aspects: incision length; operative time; blood loss; time for bed rest postoperatively; postoperative complications. compare index between two groups, The data statistics with SPSS13.0.Result: The differences in incision length, operative time and blood loss between the artificial femoral head prosthetic replacement group and the DHS internal fixation group were of no statistical significance(P>0.05),The differences in time for bed rest postoperatively, postoperative complications rate and satisfactory functional recovery rate were of statistical significance(P<0.05),The DHS internal fixation group had more postoperative complications rate and bed rest time, postoperative complications rate showed that there was no statistics difference between the age below and above 75 years in the artificial femoral head prosthetic replacement group, satisfactory functional recovery rate showed that there was no statistics difference between the age below and above 75 years in the artificial femoral head prosthetic replacement group.Conclusion: Artificial femoral head prosthetic replacement is a useful method in the treatment for unstable femoral intertrochanteric fractures in the elderly because of low complication rate, restored functions fast and earlier motion. It was limited to reflect the result accurately and overall, It still remain to perform study in large scale between in the internal fixation group and the artificial femoral head prosthetic replacement group.
