| Purpose: Gastric cancer, as one of the most common malignancy in digestive system, but the exact pathogeny of it is not completely clear. RKIP as the inhibitor of Raf-1(one of the cross-link center in many kinds of signal transduction paths),its relationship with tumor has attracted great attention of domestic and foreign scholars in recent years. This experiment is aimed at studying the relationship between the expression of RKIP and gastric cancer's occurrence and development to provide a theoretical basis for the early diagnosis and targeted therapy of gastric cancer.Methods: This study selected 152 paraffin-embedded gastric tissue slices from the recent 3-years-on-file cases of the Department of Pathology in Yan An Univisity Affiliated Hospital and also another 34 intra-abdominal metastatic lymph node tissues which corresponds to the gastric cancers with the intra-abdominal metastatic lymph node. We applied immunohistochemical SABC method to detect the expression of RKIP protein in the tissues we selected and used SPSS17.0 to do chi-square test (for the samples whose sizes are less than 40, or at least a lattice theory of frequency is less than 5, we used the chi-square test case for the continuity correction ;for the samples whose sizes are less than 40 and have a lattice theory of frequency less than 5 or the samples are less than 20 cases, we used Fisher exact probability test).We took P <0.05 as the standard of test and analyzed the diversification rule of RKIP expression from the superficial gastritis to atrophic gastritis and then to dysplasia, and finally to gastric cancer as well as the relationship between RKIP and Hp infection in order to find its internal rules.Results: The 152 cases of gastric tissue samples includ: 54 patients with chronic superficial gastritis and among these 54, 38 cases are merely superficial,10 cases with mild to moderate dysplasia,6 cases with severe dysplasia,and 19 cases with intestinal metaplasia; chronic atrophic gastritis 55 cases, among these 55,there are 32 cases are simple atrophy, 12 cases with mild to moderate dysplasia;11 cases with severe dysplasia,and 20 cases with intestinal metaplasia; 43 cases of gastric cancer, among these 43,there are 20 cases are poorly differentiated, 19 cases are moderately differentiated, 4 cases are well-differentiated,and 2 cases are mucosal carcinoma . There are 34 patients had intra-abdominal lymph node metastasis among it,and at the same time we selected the corresponding intra-abdominal metastatic lymph node tissues. In the whole 152 patients with gastric tissue specimens, RKIP protein expressed positive in 95 cases (95/152), of which 12 cases of gastric cancer tissues (12/43), simple chronic superficial gastritis 36 cases (36/38), simple chronic atrophic gastritis 27 cases (27/32), mild to moderate dysplasia 15 cases (15/22), severe dysplasia 5 cases (5/17). RKIP protein expression in 34 cases of intra-abdominal lymph node,positive is 2 cases (2/34). A total of 81 cases with Hp test results, of which 58 cases positive.In those 58 positive cases,RKIP positive expression in 53 cases (53/58), RKIP negative expression in 5 cases (5/58); and in other 23 negative cases,15 cases is positive expression of RKIP ( 15/23), RKIP negative expression in 8 cases (8/23). The result of statistical treatment is that between mild to moderate dysplasia and gastric cancer tissues the expression of RKIP was statistically significant difference,the positive expression rate of the former is higher than the latter; between mild to moderate dysplasia and severe dysplasia, the difference was statistically significant,the positive expression rate of the former is higher than the latter; between primary foci and abdominal lymph nodes the difference was statistically significant, the positive expression rate of the former is higher than the latter; between the Hp-positive cases and Hp- negative cases,the positive expression rate of the former is higher than the latter.Conclusion:, RKIP is not only as a metastasis suppressor factor but also has close correlation with the occurrence and development process of gastric cancer. For cases of severe dysplasia, the expression of RKIP has been reduced before it reached the morphological diagnostic criteria. The positive expression rate of RKIP in the Hp-positive cases is higher than the Hp-negative cases in the gastric mucosa with inflammation as the main performance, shows that in the carcinogenic process of Hp Ras / Raf / MAPK signal transduction pathway may be involved on reverse side .
