Part oneApplication of real- time tissue elastography in differentiating malignant and benign of liver tumorsObjectiveTo explore the imaging features of liver tumors with real-time tissue elastography.MethodsLiver lesions in 85 cases (female/male 9/48) were scanned with conventional ultrasonography and elastography using HI-Visinon900 system and then assessed with grade scores. Result on ultrasound were compared with those on pathology.ResultsObjective: To explor e the application value of the real-time tissue elastography in differentiating benign and malignant liver tumors.Methods: Eighty-five Liver lesions in sixty-seven cases were scaned with conventional ultrasonography ,and discrimination benign and malignant liver tumors preliminary. Then they were scaned with elastography and assessed with grade scores. Results on ultrasound were compared with those on pathology. And then exploring the imaging features of liver tumors with real-time tissue elastography.Results:Pathologic results showed that 23 of the 85 were hepatic angiosacoma, 24 were hepatocellular carcinoma (HCC),33 were Metastatic Liver Cancer (MLC)and 5 were inteahepatic cholangiocarcinoma(ICC). 20 of the 23 hepatic angiosacoma were conformity to pathologic results,3 were diagnosed as HCC.2 of 24 HCC were diagnosed as hepatic angiosacoma,20 were diagnosed as HCC and 2 were diagnosed as. 2 of 33 MLC were diagnosed as hepatic angiosacoma,6 were diagnosed as HCC and 25 were diagnosed as MLC. 1 of 5 ICC were diagnosed as HCC,4 were diagnosed as ICC. Majority of lesions with grade a~b on elastography were identified as benign on pathology, while most of masses with grade c~e on elastography were confirmed as malignant on pathology. There was statistical significance between benign group and malignant group(P<0.05) . The sensitivity, specificity and accuracy were 93.5%, 87.0%and 91.8% for elastography to detect malignant lesions. The inter-observer agreement was achieved at 91.8% lesions, and the Kappa value was 0.78.ConclusionReal-time tissue elastography of liver tumors provides a new convenient, non-invasive diagnostic methods, contribute to identify the benign and malignant live tumors. The stiffness value of three kind of malignant were higher than that of hepatic angiosacoma. And the stiffness value of MLC and ICC were higher than that of HCC, but we never found the stiffness value of ICC was higher than that of MLC.ConclusionsElastography is a promising technology and has a distinct advantage in the diagnosis of liver tumors. Part threeThe value of real-time tissue elastography in differential diagnosing liver small massesObjectiveTo investigate the value of real-time tissue elastography in the differential diagosing liver small masses.MethodsSelect 45 manignant masses at random, group 1(diameter≤2) and group 2 (>2cm) were according to the diameter of lesions on conventinal ultrasoud. Then evaluating the elastography imaging of this two group. We adopted Pearson Chi-square Test to compared the accuracy of diagnosing manignant masses between two groups.Result17 of 19 masses in group 1 were diagnosed as manignant ones,19 of 26 masses in group 2 were diagnosed as manignant ones . There was statistical significance between two groups on the accuracy of elastography imaging(P<0.05).ConclusionsElastography is not only helpful in differentiating between malignant and benign live The accuracy of elastography for diagosing small malignant(≤2cm) lesions was high.
