| Objective:To investigate the minimal erythema dose (MED) of normal skin to UVB of Dualight targeted phototherapy system, and observe its relationship to sex, age, skin types to sunlight, etc.To assess the efficacy and safety of the Dualight targeted phototherapy system combined with Wei A Lu Tincture in the treatment of vitiligo.Methods:73 healthy volunteers and 35 patients with vitiligo in Nanjing were enrolled, whose normal skin was tested by UVB of Dualight targeted Phototherapy System UV120-2.44 patients with vitiligo were divided into two groups randomly,23patients with 47 patches were treated with UVB of Dualight targeted phototherapy system twice a week and Wei A Lu Tincture once daily for 8 weeks, and the other 21 patients with 49 patches were treated with only UVB of Dualight targeted phototherapy system twice a week for 8 weeks as control. All the patches were photographed and imitated before the treatment, and after the fourth, eighth, twelfth and the last treatment. The imitations were scaned into computer and the area of them were calculated by computer digital image analysis(CDIA). Meanwhile, the adverse effects were recorded.Results:The MED value was (189.17±56.156) mJ/cm2 (range:90~330 mJ/cm2)among all volunteers and patients. It was (155.88±34.996)mJ/cm2 (range:90~210mJ/cm2)in skin typeⅢ, and (218.95±54.957)mJ/cm2 (range:120~330mJ/cm2) in skin type IV.The MED value in skin type III was significantly lower than that in skin type IV (P<0.01). Among males, the MED value was (154.29±39.443) mJ/cm2 (range:90~210mJ/cm2) in skin type III, and (224.4±54.854) mJ/cm2 (range:150~330mJ/cm2) in skin type IV.Among females, the MED value was (157±32.179) mJ/cm2 (range:90~210mJ/cm2) in skin typeⅢ, and (214.69±55.532)mJ/cm2 (range:120~330mJ/cm2) in skin typeⅣ.The MED value in skin type III was significantly lower than that in skin typeⅣamong both males and females(P<0.01). There was no difference of MED value between males and females, or among four age groups(P>0.05).After all the 8 weeks treatment, the total effective rate in treatment group (78.72%) was significantly higher than that(59.18%) in the control group(P=0.039).The area of patches was decreased by 0.73±0.227(range:0.25~1) and 0.60±0.290(range:0.12~1) in treatment group and control group separately, and there was a statistical difference(P=0.023). In treatment group, the area decrease rate of patches(ADRP) in face(P=0.014) and neck(P=0.028) were both significantly higher than that in hand-foot. There was a significant difference between the two groups in ADRP in neck(P=0.039), but no difference in ADRP in other positions. The area decrease rate of patches whose disease course were more than 1 year in the treatment group was significantly higher than that in control group(P=0.002),and ADRP in progressive stage was also significantly higher than that in control group(P=0.005). Adverse effects happened in all the 96 patches in the initial several treatments, but they all got released after treatments, and the total 44 patients completed all 8 weeks treatments.Conclusions:It is indicated that UVB-MED value of Dualight targeted Phototherapy System UV120-2 is mainly dependent on the skin types to sunlight, but it is not related to sex and age distributions. The Dualight targeted phototherapy system and Wei A Lu Tincture are effective in the treatment of vitiligo without obvious adverse effects. Computer digital image analysis(CDIA) is good at assessing curative effect of vitiligo by calculating the area of patches accurately. |
