Objectives: The thyroid carcinoma which accounting for about 1% of systemic malignancy is the most common malignant tumor of thyroid. Except medullary carcinoma, the vast majority of thyroid cancer derives from follicular epithelial cells. Among the patients of the thyroid carcinoma women more common and the incidence of disease is rising in recent years, the vast majority occurred in young adults. Because its clinical symptoms are not obvious, its typical morphological characteristics can not be found by the phantom study inspection (such as the B-mode ultrasound examination), it was often misdiagnosed as benign lesions, and especially the rate of differentiated misdiagnosis is relatively high. The thyroid carcinoma transfer earlier than others and the rate is high. When the primary tumor can not be touched, then the cervical lymph node metastasis could be occurred. Surgery is the commonest and reliable means of treatment. Misdiagnosis of malignant thyroid nodules may delay diagnosis and treatment and non-standard surgical operation will bring about suffering and economic burden for patients. Therefore, in the process of surgery using the right way is critical. This thesis analyzed and summarized the reports of surgical treatment of thyroid cancer, through by1979 which from the network database (Chinese Journal Full-text databases, Ovid foreign-language text databases, etc.), and explore how to choose the best surgery in the process of surgical treatment of thyroid cancer in order to offer some references for clinical treatment.Methods: A retrospective analysis of the clinical records of 382 patients was analyzed for the method of surgery treatment for thyroid carcinoma from resection extent,operative techniques,intraoperative and postoperative complications.Results: 367 patients accepted surgery.10 patients (4.9%) of the 201 patients (54.8%) which accepted total thyroidectomy (TT) had transient or short- term completely recovered hypocalcemia postoperatively,and there were no cases with injury of recurrent laryngeal nerve;6 patients (4.6%) of 131 patients (35.7%) which accepted unilateral lobectomy had transient hypocalcemia postoperatively,and there were no cases with injury of recurrent laryngeal nerve;4 examples (11.4%) of recurrent patients which were 35 examples (9.5%) had one- sided recurrent laryngealnerve injury and temporary hypoparathyroidism in recurrent patient, and one of the cases had permanent hypoparathyroidism. Thyroxin were used for all the patients after surgery.Conclusions: In the thyroid carcinoma patient surgery course of treatment, total thyroidectomy (TT) can avoid the intraoperative and postoperative complications greatly by meticulous capsule dissection technique and it can reduce the recurrence rate of thyroid carcinoma.Total thyroidectomy could be served as the routine operative method for thyroid carcinoma.
