Objective: Chronic Obstructive Pulmonary Disease (COPD), because of its large number of illness, high mortality and heavy social burden, has become an important public health problem. COPD ranks No.4 cause of death worldwide, World Bank / World Health Organization announced, COPD will rank No.5 in the economic burden of disease worldwide until 2020. In China, COPD seriously endanger the people's health as a chronic respiratory disease, survey among some parts of China, COPD prevalence in people over 40 years of age accounted for 8.2%. The pathogenesis of COPD is still unknown which is now accepted that airway inflammation is the main pathogenesis of COPD. Nuclear factor-κB (NF-κB) participated in gene transcription of various inflammatory cells and immune cells. This experiment studies the level of NF-κB in the peripheral blood mononuclear cell nucleus of acute exacerbation stage of COPD patients (AECOPD) and severe-slightly severe AECOPD patients with glucocorticoid treated, then compare the expression level of NF-κB in the peripheral blood mononuclear cell nucleus with interleukin-8 (IL-8) and interleukin-10 (IL-10) in serum, and compared the levels of NF-κB in the peripheral blood mononuclear cell nucleus with lung function of AECOPD patients, To explore the effect of NF-κB on the pathogenesis of COPD and the correlation between the expression level of NF-κB with the severity of COPD and the possible mechanism to glucocorticoid treating COPD.Material: Set AECOPD patients as observation group with 60 observable objects, divided it into two subgroup according to their condition: Light- moderate group of 30 cases, severe-slightly severe group of 30 cases, the same basic treatment (for anti-infection, absorbing oxygen, relaxing the bronchial smooth muscle, relieving cough and other symptomatic treatment) was given, added glucocorticoid to the severe-slightly severe group of AECOPD patients. Set the healthy over the same period as control group with 30 observable objects. Using western blotting method to detect the level of NF-κB in peripheral blood mononuclear cells nucleus, the level of IL-8 and IL-10 in serum were detected by using ELISA assay, and detected the pulmonary function (FEV1 percentage of predicted value %, FEV1/FVC value) with AECOPD patients.Results:1 The level of Nuclear factor-κB in peripheral blood mononuclear cells nucleus of AECOPD patients1.1 The level of NF-κB in peripheral blood mononuclear cells nucleus of AECOPD patients (4.37±1.89) was higher than the normal control group (1.70±0.39) (P<0.01).1.2 The level of NF-κB in peripheral blood mononuclear cells nucleus of severe-slightly severe AECOPD patients (5.87±0.28) was higher than the light-moderate group (2.75±0.95) (P< 0.01).1.3 The level of NF-κB in peripheral blood mononuclear cells nucleus of severe-slightly severe AECOPD patients treated by glucocorticoid (2.52±0.42) was lower than the group of patients before treated (5.87±0.28) (P<0.01), but still higher than the normal control group (1.70±0.39) (P< 0.01).2 The level of IL-8 in serum of AECOPD patients2.1 The level of IL-8 in serum of AECOPD patients (9.18±13.96)pg/ml was higher than the normal control group (35.87±6.35)pg/ml (P<0.05).2.2 The level of IL-8 in serum of severe-slightly severe AECOPD patients (52.89±10.97)pg/ml was higher than the light-moderate group (40.99±5.54) pg /ml (P<0.05).2.3 The level of IL-8 in serum of severe-slightly severe AECOPD patients treated by glucocorticoid (40.87±5.97)pg/ml was lower than the group of patients before treated (52.89±10.97)pg/ml (P<0.05), then compared with the normal control group (35.87±6.35)pg/ml, there was no statistically significant (P>0.05).3,The level of IL-10 in serum of AECOPD patients3.1 The level of IL-10 in serum of AECOPD patients (82.91±3.83)pg/ml was lower than the normal control group (148.72±2.52)pg/ml (P<0.01).3.2 The level of IL-10 in serum of severe-slightly severe AECOPD patients (52.98±1.30)pg/ml was lower than the light-moderate group (113.68±2.36)pg/ml (P<0.01).3.3 The level of IL-10 in serum of severe-slightly severe AECOPD patients treated by glucocorticoid (102.00±2.88)pg/ml was higher than the group of patients before treated (52.98±1.30)pg/ml (P<0.01), but still lower than the normal control group (148.72±2.52)pg/ml (P< 0.01).4,The correlation between the level of NF-κB in peripheral blood mononuclear cell nucleus with the level of IL-8, IL-10 in serum of AECOPD patientsThe level of NF-κB in peripheral blood mononuclear cell nucleus increased with the level of IL-8 in serum increasing, show a positive correlation trend, the correlation coefficient r=0.84; The level of NF-κB in peripheral blood mononuclear cell nucleus decrease with the level of IL-10 in serum increasing, show a negative correlation trend,the correlation coefficientr =- 0.78.5,The correlation between the level of NF-κB in peripheral blood mononuclear cell nucleus and pulmonary function with AECOPD patientsThe level of NF-κB in peripheral blood mononuclear cell nucleus decreased with the FEV1 percentage of predicted value % increasing, show a negative correlation trend, the correlation coefficient r=- 0.97.Conclusions:1,NF-κB participated in airway inflammation response of AECOPD,it,s an important indicator to disease severity,Its an important link of AECOPD waterfall inflammatory response.2,IL-8 participated in airway inflammation response of AECOPD,it,s regulated by NF-κB,When the inactive NF-κB in the cytoplasm was stimulated by inflammatory injury or immune stimulation,it entered into the nucleus and started gene transcription of IL-8, promoted the expression level of IL-8. but the specific adjustment process remains to be further explored.3,IL-10 can inhibit the airway inflammatory response of AECOPD, IL-10 controled the inflammation response of AECOPD by inhibiting the expression level of NF-κB. 4,Glucocorticoid can inhibit airway inflammation reponse. It can promote the expression of IL-10 in serum and thus to Inhibit the expression level of NF-κB and IL-8 to achieve controlling inflammation response of COPD. Besides glucocorticoid also participate in a varity of inflammatory response of AECOPD,remains to be further explored.
