OBJECTIVE:Septic shock is one of the most important cause of death in Intensive Care Unit. In the Septic shock patients, if they combined with disease in central nervous system, they would face with a worse prognosis. Because of the unknown immunity mechanisms for central nervous system, the fatality of septic shock patients has not decrease markedly. In many of the mechanisms, TLR4 was confirmed that it was important to the immune in central nervous system, and it was a key part to the immune of G(-) bacterium. Some researchs confired that:we can decrease the fatality through decrease the the Expressions of NF-κB.In the Septic shock patients who combined with disease in central nervous system need to use sedative drugs usually. Propofol is the most important sedatives during the process of anesthesia and both in Intensive Care Unit. It was confirmed that propofol had brain protective function while doctors used to sedative. Existing mechanisms concentrated on the inflammatory factors and there was no research report the mechanisms. So, this research want to explain the brain protective mechanism through detecting the effects of propofol on the expressions of TLR4 and NF-κB P65 in rabbitbrainbymolecular biologymethods. METHODS:40 male rabbit were randomly divided into four groups: control group (group C), shock 1hour group(group LS1h), shock 2 hour group(group LS2h) and shock and propofol group(group LSP). Every group included ten rabbits. The rabbits were kept spontaneously breathing through incision of trachea after anesthesia. And then exposure the carotid artery to monitor the MAP, exposure the vena cervicalis to transfuse and inject drugs. In group LS1h, group LS2h and group LSP,0.5mg/kg LPS was injected slowly to build the endotoxic shock model, while 5ml saline was injected in group C. We identified the MAP lower than 60mmHg as the standard of endotoxic shock. Record the MAP at these time-point:T0, T1, T2, T3, T4, T5, T6, T7. The rabbits were killed at T6 in group LS1h and at T7 in other groups. The whole brain tissue was put in a ice box to detect TLR4 and NF-κB P65.RESULTS:1.Compared with group C, the expression of TLR4 and NF-κB P65 in group LS1h, group LS2h and group P increase significantly.2. Compared with group LS1h, the expression of NF-κB P65 in group LS2h increase significantly.3. Compared with group LSP, the expression of NF-κB P65 in group LS2h increase significantly. CONCLUSION:1. In the brain of endotoxic shock rabbit, the expression of TLR4 increase at the earlier period. So we should treat as2.Propofol can decrease the expression of NF-κB P65 in the brain of endotoxic shock rabbit, this maybe the real mechanism of the protective function of brain.
