Objective: This study aimed at investigating the expression of hypoxia inducible factor-1α(HIF-1α)and p53 and determine their relation to clinicopathological variable in human clear cell renal cell carcinoma(ccRCC). Furthermore, we investigate the effect on prognosis. Methods: The expression of HIF-1αand p53 protein were detected by the immunohistochemical methods(sterptavidin/Peroxidase)in nephrectomy specimens obtained from 65 patients with ccRCC and 10 specimens of normal renal tissue. Compare the expression of HIF-1αand p53 in clinicopathological variable of ccRCC, Multivariate analysis was performed according to Cox proportional hazard model. Results:1. The positive rate of HIF-1αand p53 protein staining was recognized in 40.0%(26/65)and 52.3%(34/65)cases, respectively. but negative expression in normal renal tissue specimens(P<0.05). 2. The positive expression of HIF-1αwas associated with clinical stage,histological grade and lymph node metastasis of ccRCC(P<0.05), but significant correlation were not found in the sex,age and tumor size(P>0.05). The positive expression of p53 was not associated with above clinicopathological variable of ccRCC(P>0.05). 3. Significant correlation were found among the expression of HIF-1αand p53 in patients with ccRCC(r=0.416, P<0.05). 4. Univariate survival analysis indicated that prognosis of patients with ccRCC was significantly correlated with age of patient,tumor size,clinical stage,lymph node metastasis and expression of HIF-1a (P<0.05). Cox's proportional hazards model multi-variate survival analysis showed that tumor size,clinical stage and expression of HIF-1a may be independent prognostic factors of ccRCC. Conclusion: 1. HIF-1αand p53 were both positive expression in patients with clear cell renal cell carcinoma(ccRCC), might be connected to the occurrence and development of ccRCC. 2. The positive expression of HIF-1αwere associated with clinical stage,histological grade,lymph node metastasis and prognosis of ccRCC, but significant correlation were not found in the sex,age and tumor size. The positive expression of p53 were not associated with above clinicopathological variable of ccRCC. 3. Significant positive correlation were found among the expression of HIF-1αand p53 in patients with ccRCC. 4. Cox's proportional hazards model multi-variate survival analysis showed that tumor size,clinical stage and expression of HIF-1a may be independent prognostic factors of ccRCC;The positive expression of HIF-1αwas related to poorly prognosis. 5. The positive expression of HIF-1αwas associated with invasion and metastasis of ccRCC, immunohistochemical staining for HIF-1αmay be an important indictor in evaluating the biological behavior of ccRCC such as invasion,metastasis and prognosis and a new target of anti-tumor therapy.
