| Teacher Wang Huanlu has now engaged in medical practice for more than 50 years, has accumulated rich clinical experience. In the year of 2008 and 2012, he was respectively selected as a tutor of which is the work of the fourth and the fifth batch of the famous traditional Chinese medicine expert’s academic thought and clinical experience to inherit. Teacher Wang Huanlu read in the classics, learn waidely from other’s strong points, combined with years of clinical practice, gradually formed its own unique academic thoughts and clinical diagnosis and treatment experience. This study through interviews with Mr.Wang, is to understand his experience of learning Chinese medicine,and to dig out the source of the formation of Mr. Wang’s academic thought. The purpose of this study is to understand through the experience of Mr. Wang to study Chinese medicine. To understand and grasp his main academic context, research on the origin and development of his academic thought formation, refining the essence of his academic thought. This study sum up the clinical experience of Mr. Wang by collecting the data on the clinical treatment of the disease. To observe the Jiajian Yiyizhuye decoction to treat the distention and fullness which type is damp heat in the spleen and the stomach, to explore the clinical effect. This thesis mainly consists of three parts as follows.1. Origins Overview about Wanghuan Lu teacher Academic ThoughtThe first part through interviews, understand the teacher Wang Huanlu’s growth environment, clear his family background and learning Chinese in the background, to explore the learning experience of traditional Chinese Medicine. Thus, we can know the relationship between Mr. Wang learning experience of Chinese medicine and its future academic thought. In addition, by the classical medical books explore the theory of traceability, carding Teacher Wang’s "heavy stomach, spleen and stomach protection" and "pay attention to hot and humid" theory of the origins thought. Specifically, The formation of Wang’s spleen and stomach" academic thought, from the "classic" and "typhoid" in the spleen and stomach is discussed. He inherited the theory of spleen and stomach. Ye tianshi stomach yin theory and other famous on spleen and stomach theory based, and gradually developed into teacher’s academic thought of the spleen and stomach. The formation of Mr. Wang’s "pay attention to hot and humid" thought related with experience of background and his father began to education. In addition, it also stems from the theory of heat "Yellow Emperor", "typhoid", especially influenced by the theory of the science of seasonal febrile diseases.2.The research of teacher WangHuanlu’s academic thoughts and clinical experienceTeacher Wang insists on the concept of dialectical treatment of the syndrome differentiation in clinic. Treatment based on syndrome differentiation is an analysis of the diagnosis and treatment of disease thinking, which has the most dynamic and characteristic in TCM theory. It is the essence of TCM. In addition,Mr. Wang also advocates the combination of disease differentiation treatment concept. He pointed out that in collecting clinical data, in addition to the "four diagnosis " can also increase the "reference" diagnosis which means to refer to the patients of the modern medical objective examination report. This method combined with disease differentiation, syndrome differentiation, etiology and pathogenesis in order to more accurately differentiate patients and then be treated. On the academic thoughts, teacher WangHuanlu emphasizes the thought of "paying attention to the spleen and stomach". This article is discussed the importance of the ideas and specific protection methods of spleen and stomach from the disease prevention and treatment, nursing. To treat disease, Mr. Wang attaches great importance to the pathogenic Qi in the position and role of disease.He advocates to "eliminate pathogenic for all". By analyzing the relationship between healthy Qi and pathogenic Qi, This paper discusses the status, pathogenic role in the disease, and combes the clinical experience of eliminating pathogenic treatment.Teacher WangHuanlu has abundanted clinical experience, mainly summarized the following several aspects. First, the article introduces teacher Wang’s experience on the diagnosis and treatment.method. Second, to summarize several characteristics treatment experience in clinical which belongs Teacher Wang, including of summer-heat invading the lung, asthma attack stage, reflux esophagitis, chronic constipation, promoting blood circulation to remove blood stasis method, Biqiu, rheumatoid arthritis, metrorrhagia and metrostaxis, pediatric anorexia. 3.3.The clinical curative effect observation is about of applicating JiajianYiyizhuye decoction to treat epigastric fullness which type is spleen stomach damp heat.Purpose:Under the guidance of TCM theory and according to many years clinical experience of Mr.Wang,to study the clinical curative effects of JiajianYiyizhuye decoction which was used to treat epigastric fullness of spleen stomach damp heat type.Method:Select 45 patients met the diagnostic criteria for the traditional Chinese medicine clinic fullness (spleen-type) and Western medicine diagnosis standard diagnostic criteria for functional dyspepsia in Teacher Wang’s outpatient which were included in the treatment group. Select 17 patients met the diagnostic criteria for the traditional Chinese medicine clinic fullness (spleen-type) and Western medicine diagnosis standard diagnostic criteria for functional dyspepsia in the Zhanlanlu Community Health Center general clinic which were included in the control group.The treatment group was treated with modified JiajianYiyizhuye decoction, control group received domperidone 10mg Tid treatment. Observe 28 days, and record the change of the symptoms, syndrome before and after the treatment. After treatment, clinical efficacy, TCM syndrome and improvement of symptoms were evaluated,and carried out monitoring of adverse reactions.Results: At the 14 day of treatment, the treatment group of epigastric fullness, bad breath, mouth parched and tongue scorched were improved than control group (P<0.05). But for reducing diet, early satiety, belching, sticky stool symptoms between the two groups had no significant difference (P> 0.05). At the 28 day of treatment,the treatment group of dry mouth, bad breath and stool sticky greasy symptoms improved better than the control group (P< 0.05). But for epigastric fullness, reducing diet, early satiety, belching symptoms had no significant difference between the two groups (P> 0.05). Self controlled before and after treatment, two groups of the epigastric fullness symptom score changes were significantly different (P<0.01), indicating the treatment method and the control group used treatment methods are all effective therapies. Before and after treatment,epigastric fullness between two groups integral rate of change in symptoms over comparison,14 days tips have obvious differences between the two groups (P< 0.01), described that the treatment group improved epigastric fullness of symptoms better than the control group; between the two groups has no significant difference (P> 0.05) after treatment 28 days, two treatment methods described in this point in time the treatment of symptoms of epigastric fullness was no difference in efficacy. At 14 days, the syndrome of treatment group to improve curative effect is better than that of control group (P< 0.01). At 28 days, no significant difference between two groups(P> 0.05). Before and after the treatment group before and after contrast, significant difference between the two groups of TCM syndromes were (P< 0.01). It shows that the treatment group and the control group with the treatment methods are effective for the treatment group. Before and after treatment between the two groups of TCM syndrome integral comparison of the rate of change indicates a significant difference between the two groups (P< 0.01).It shows that the treatment group was significantly better than the control group of syndromes.Conclusion:JiajianYiyizhuye decoction is an effective method for the treatment of type of damp heat in spleen and stomach fullness, and clinical symptoms and epigastric fullness improved with more rapid than the group of domperidone,and the curative effect is reliable. Through the clinical efficacy of the study, our clinical practice provides a new treating spleen stomach damp heat syndrome and fullness effective prescription, It is worth to fellow promotion. |
