Objective:Incidence of post-adolescent acne is related to such factors as negative emotion, sleep, preference for oily and sweet food, oil skin, menstrual disorder, family history, age, cosmetics and the like. Currently, the relationship between chronobiology disorders and post-adolescent acne is gradually taken seriously, but are just limited to preliminary clinic observation, and the exact mechanism is still unclear. This research aims to screen major related factors from possible related factors such as chronobiology disorders including sleep, age, family history, menstrual history, skin type and so on through the large sample research, to focus on the effect of sleep of chronobiology disorders on post-adolescent acne, and also to offer a new way and theoretical reference that can help to guide the sleep of adult female with acne and control of acne.Methods:By adopting case-control study and cross-sectional study, and designing case questionnaire, the data is gathered about 125 cases of adult women with acne from Dermatology Clinic of Oriental Hospital, Beijing University of Chinese Medicine and 30 cases of adult women (25 to 45 years old) who may become pregnant, including general information and PSQI(pittsburgh sleep quality index), and then adopt laboratory examination (the level of sex hormones) during the second to the fifth day of menstruation; case group also need to gather the condition of acne, TCM syndrome. By applying Locistic regression analysis, to screen the disease risk factors of post-adolescent acne’s chronobiology disorders, and to analyze the effect of risk factors on post-adolescent’s TCM Syndrome Type and the level of sex hormones of all the research object. Additionally, compare case group with control group, and compare the level jof hormones between various TCM Syndrome Type group and contrast group just to define the general level of hormones of post-adolescent acne and the character of various Syndrome Type of sexhormones’s level.Results:1.among the included 125 cases of post-adolescent acne,the majority are inage from 25 to 28 years old (43.33%), secondly range from 29 to 32(30.00%), the least are in 40 or more years old. Patients with family history account for 48.80%, patients with menstrual disorder account for 24.00%; the major character of post-adolescent acne is Yin-Deficiency and Fire-Hyperactivity Syndrome(50.40%), followed by Spleen Deficiency and Dampness-Heat Syndrome(33.6%).2.After Logistic regression analysis, family history, greasy faces, bed time and sleep duration of chronobiology disorder factors are the risk factors of post-adolescent acne.3. Compared with the average in terms of the sex hormones, for post-adolescent acne, E2 decreases while T and T/E2 increase.some with Yin-Deficiency and Fire-Hyperactivity Syndrome show the decrease of E2 and higher T and T/E2 than the average; some with Spleen Deficiency and Dampness-Heat Syndrome show the normal level of E2 and increasing level of T and T/E2; some with gastrointestinal damp-heat Syndrome display normal level of E2 and T/E2 and the increasing level of T.4. analyzing risk factors of post-adolescent acne demonstrates that family history and greasy face have no effect on TCM syndrome; late sleep has close relationship with Yin-Deficiency and Fire-Hyperactivity Syndrome; family history and sleep deprivation have no effect on E2, T and T/E2; sleep time has significant effect on E2 and T/E2:the later sleep time is, the lower Estradiol level is, the higher T/E2 level is, having no relationship with T; skin level has no relationship with E2 and T/E2, but has relationship with Testosterone level. In general, the more greasy the face is, the higher Testosterone level is.5. Post-adolescent acne are predominantly characterized by midrange acne, accounting for 82.4%.level acne of four Syndrome Types and distribution of lesion part show no difference.Conclusion:1. Of chronobiology disorder factors, sleep time, sleep duration, family history, and greasy faces are the risk factors of post-adolescent acne.2. From the aspect of TCM etiology and pathogenesis, the late sleep time may trigger Yin-Deficiency and Fire-Hyperactivity Syndrome; from the aspect of western medicine’s pathogenesis, the late sleep time may lead tothe decrease of E2 level and increase of T and T/E2, thus inducing acne.3. Yin-Deficiency and Fire-Hyperactivity Syndromeis the major syndrome of post-adolescent acne, and such syndrome are also characterized by the decrease of E2 level and increase of T and T/E2.
