Osteoporosis (OP) and its induced fracture was serious damage to the health of elderly. With the increase of elderly population proportion and the extension of life expectancy, the incidence of osteoporosis increase year by year. While the worst consequence of osteoporosis is osteoporotic fracture, it is the most common cause for older people lossing of self-care ability and being disability. How to promote the osteoporotic fracture healing, improve recovery expectations, is the urgent need to solve clinical problems.Pulsed electromagnetic field (PEMF) has been used in the prevention or treatment of delayed union of fracture and bone nonunion for years. A large number of animal experiments and clinical studies have confirmed PEMF can promote fracture healing. While, application PEMF to intervene in osteoporosis and the attempt of osteoporotic fractures are rarely reported at home and abroad.Method in the clinical treatment of osteoporosis is not much at home and abroad, the main treatment is medication for a long time, it has higher cost, drug side effects, curative effect is not obvious. In patients with osteoporotic fractures and fracture is loose, reduction and internal fixation surgery treatment the effect not beautiful, easy to fracture piece is difficult to reduction and fixation, internal fixation loosening, slippage, and so on and so forth, patients need to be in bed for a long time, after injury prone to neuromuscular function degeneration, bedsore, urinary tract infections, hypostatic pneumonia, lower extremity deep vein thrombosis and other complications, as well as the high morbidity. Once in bed for a long time, the mortality rate can be as high as astonishing 25-35%. Whether in the hospital surgery and conservative treatment, or home care, rehabilitation, is very difficult. Therefore, how-to treat osteoporosis, especially how to promote osteoporotic fracture healing need to cause the attention of medical scientific research workers and thinking.Object:By female rat model of osteoporosis animal experiment and model of rat bone marrow mesenchymal stem cells in vitro experiment, verify the pulse electromagnetic fields (PEMF) has a role for the treatment of osteoporosis and promoting fracture healing.Method:Resect bilateral ovaries to bulit osteoporosis model, set the kirschner wire into the bone marrow cavity and then make a fracture of femoral shaft. Then take the PEMF treatment(6 hours per day), take regular X line to observe fracture callus formation. Culture the BMSC in vitro, observe the effect of PEMF on BMSC for its cell type, growth characteristic. Analyze the influence of PEMF on osteogenic differentiation for BMSC through the ALP assay.Result:By observing the X-ray, the fracture in the PEMF stimulating group, healed a little earlier than the non-simulating group. And the callus formation condition is significantly better than the non-simulating group. Apply MTT colorimetric method to draw growth curve, analysis the growth activity of the third generation of BMSCs. The BMSC from the osteoporosis froup, culture slower than the BMSC from common rat. The BMSC got PEMF stimulation culture faster and better than the control group. The BMSC got from the rat which got PEMF stimulation for 6 months culture faster than the BMSC got from the same rat with no PEMF stimulation. There were significant statistical differences (P<0.05) between all the groups. The cell type results show that BMSC which get PEMF stimulation assumed more cell on S-phase, there were significant statistical differences (P<0.05) between all these 3 control trials. ALP assay results show the BMSC got PEMF stimulation will make more ALP than the non-stimulation group, there was significant statistical difference (P<0.05)Conclusion:Animal experiments, PEMF stimulate fracture directly, can promote the osteoporotic fracture heal ing; In vitro experiment, PEMF stimulate target cells, can improve the activity of BMSCs growth, improve the ability of proliferation and osteogenic differentiation.
