Study On The Treatment Of Secondary Inflammatory Reaction Of Acute Severe Cholangitis With Fuzheng Qingre Zhuyu Decoction

Purpose:To observe the curative effect of righting stasis Chinese medicinal herbs combined with endoscopic treatment of acute cholangitis of severe type secondary to systemic inflammatory response syndrome.T lymphocyte subpopulation include CD4+,CD8+,CD4+/CD8+ and NK cells,and PCT、IL-6 were combined to investigate the mechanism of treatment.Methods:45 cases diagnosed as ACST secondary SIRS were randomly divided into three groups: clearing heat and detoxicating, righting stasis and control group. Every group has 15 cases. Before the treatment、remove the obstruction after 3 and 7 days after the obstruction test serum white blood cell count(WBC); interleukin-6(IL-6); procalcitonin(PCT). Before the treatment and remove the obstruction after 7 days after the obstruction test serum T lymphocyte subpopulation(CD4+、CD8+、CD4+/CD8+) and natural killer cell.Results:Group A, B, C respectively 1 person, 1 person, 2 people were excluded completed the study, other patients completed the trial.On the 7th day after operation, WBC : A group:9.71±1.72×109/L(complete with C group, P>0.05);B group:7.35±1.40×109/L(complete with C group,P<0.05);C group:9.92±2.01×109/L.IL-6: A group:22.89±11.05 ng/L(complete with C group, P>0.05);B group:13.99±4.85 ng/l(complete with C group, P<0.05);C group:28.72±13.63 ng/l.PCT: A group:1.29±0.94 ng/ml(complete with C group,P>0.05);B group:0.80±0.41 ng/ml(complete with C group, P<0.05);C group:1.66±0.76 ng/ml.CD4+: A group:36.90±3.44%(complete with C group, P>0.05);B group:39.72±2.89%(complete with A group, P<0.05,complete with C group, P<0.05);C group:36.48±2.98%;CD8+: A group:30.41±3.28%(complete with C group, P>0.05);B group:27.41±2.11%(complete with A group, P<0.05,complete with C group, P<0.05);C group:30.11±4.42%;CD4+/CD8+: A group:1.22±0.19(complete with C group, P>0.05);B group:1.46±0.18(complete with A group, P<0.05,complete with C group, P<0.05);C group:1.23±0.20.NK cell: A group:13.44±3.10% (complete with C group, P>0.05);B group:15.11±2.56%(complete with A group, P>0.05,complete with C group, P<0.05);C group:13.05±1.65%.Conclusion:Clearing heat joint centralizer by stasis of Chinese medicine in secondary SIRS athabasca have auxiliary therapy effect, mechanism may be through Reduce proinflammatory mediators to reduce the inflammatory cascade reaction,and improve the body’s immunity to speed up the recovery of infection.
