Objectives:To discuss the value of biexponential IVIM-DWI MRI in preoperative diagnosis of pancreatic carcinoma..Material and Methods:The object of study is patients who underwent magnetic resonance imaging and pathological diagnosed pancreatic cancer in our hospital from June 2014 to February 2016,the total number is 40 cases, the patients were 24 cases of male,16 cases of female, Patients aged from 42 to 79 years old, average age 66±9.0years,All subjects underwent IVIM(b value was set to 0、30、50、150、200、300、400、500、800、1000、1500s/mm2) sequence of the abdominal conventional MRI sequence. All examinations were taken in the MR (GE Discovery MR7503.0 T) machine.All data were input to the AW 4.6 workstation, then were postprocessed by two radiologists who had a lot of experience. layers of large size tumor were choiced for putting three region of interests (ROI), and get the average. In control group,We choosed three areas of interest in the pancreatic tissue which were not invaded, take its average. biexponential IVIM model is applied to calculation of Slow apparent diffusion coefficient(Slow-ADC),Fast apparent diffusion coefficient(Fast-ADC) and fraction of Fast apparent diffusion coefficient(f) and get the corresponding pseudo color pictures.To comparison pancreatic tumors and the normal pancreatic tissue pancreatic tumor lesions not involved region of interest (ROI) IVIM parameters differences by independent sample t-test, explore pancreatic tumors and pancreatic tumor uninvolved normal pancreatic tissue interested area inland molecular diffusion condition and perfusion differences, aimed at improving the biexponential IVIM-DWI parameters on pancreatic cancer preoperative diagnostic value.Result:The data calculated from IVIM sequences showed that the value of Slow-ADC of pancreatic tumor lesions not involving the normal pancreatic tissue and pancreatic carcinoma tissue is ((0.661±0.231)×10-3mm2/s、0.599±0.132×10-3mm2/s),has no statistically difference and while the value of Fast-ADC respectivly is ((51.72±10.16)×10-3mm2/s、(40.36±8.17) ×10-3×10-3mm2/s) has statistically difference; the value of f is (0.336±0.076、0.282±0.103) has statistically difference between pancreatic tumor lesions not involving the normal pancreatic tissue and pancreatic carcinoma tissue.Conclusion:The parameter of IVIM-DWI can reflect the diffusion of pure water molecules in target tissue, Fast-ADC and f value reflects the microcirculation perfusion, among them in tissue, the Slow-ADC value in pancreatic carcinoma and pancreatic tumor lesions not involving the normal pancreatic tissue had no statistical significance (p>0.05). there are statistical significance (p<0.05) of pancreatic cancer and pancreatic tumor lesions not involving the normal pancreatic tissue in Fast-ADC and f value of the IVIM parameter.Objectives:To discuss Explore the correlation of biexponential IVIM-DWI for primary lesions of pancreatic carcinoma and intrahepatic metastasis of pancreatic cancer, Through the correlation of the parameters, it can improve the imaging diagnosis of intrahepatic metastasis of pancreatic cancer by using the double exponential model IVIM-DWI.Material and Methods:The object of study is patients who were diagnosed pancreatic carcinoma,Combined with clinical history, laboratory examination, CT and MRI imaging method were diagnosed intrahepatic metastasis of pancreatic cancer patients (excluding other tumor metastasis and primary liver history) underwent magnetic resonance imaging and Puncture pathological diagnosed pancreatic cancer in our hospital or other hospitals from June 2014 to February 2016,the total number is 49 cases,28 cases of male,21 cases of female, Patients aged from 45 to 86 years old, average age 70±9.9years. The patients were all subjects underwent IVIM(b value was set to 0、30、50、150、200、300、400、500、800、1000、1500s/mm2s/mm2) sequence besideds of the abdominal conventional MRI sequence. All examinations were taken in the MR (GE Discovery MR7503.0 T) machine.All data were input to the AW 4.6 workstation, then were postprocessed by two radiologists who had a lot of experience. Three layers were choiced for putting three region of interests (ROI), in region of layers of large size tumor in pancreatic carcinoma and Intrahepatic metastatic lesions were choiced. And take its average,All parameter values were recorded. Slow-ADC, f, and Fast-ADC were calculated by biexponential IVIM-DWI,We can get pseudo-color images Prameters derived from pancreatic carcinoma and liver metastatic lesions were compared by analysis of correlation by pearson of SPSS 17.0. By comparing the correlation of both IVIM related parameter value to improve the diagnosis of pancreatic cancer move to liver.Result:The data calculated from IVIM sequences showed that the value of Slow-ADC in pancreatic carcinoma and liver metastatic lesions respectively were((0.550±0.154)×10-3mm2/s、 (0.535±0.164)×10-3mm2/s) has statistically difference with obvious correlation and the Fast-ADC value were (3.515±1.967)×10-3mm2/s、(4.27±2.633)×10-3mm2/s and the value of f 0.326±0.0852、0.372±0.113,has weak correlation.Conclusion:The Slow-ADC parameters of IVIM DWI in the pancreatic carcinoma and liver metastatic lesions has statistically significant difference with middle intensity correlation, Fast-ADC and f values in the the pancreatic carcinoma and liver metastatic lesions had weak intensity correlation.
