Objective: Study on the out-patients and hospital diabetes patients'information, purpose to learn of behavior related with disease, influencing factors for medical costs and utilization of health services. The study provides the evidence and proposal for developing diabetes disease management approach.Methods: Beijing, Guangzhou and Wuhan are choosen as study field. 1704 diabetes patients are samples through cluster sampling method. Survey of households includes basic data of patients, blood glucose control, out-patient treatment and center for community health service in 2007. Information of medical insurance are collected, including out-patient and hospital cost.Result:1. Behavior related with disease: blood glucose monitoring is not good, 42.35% of patients'blood glucose up to standard. 17.31% of patients can keep regular blood glucose monitoring. The majority of patients demonstrate that they do weight control, stoping smoking, stoping drinking, taking exercise and diet control.2. Medical cost: influencing factors for outpatient costs are gender, education, income, complication, the choice of medical institutions. The cost of western medicine plus examination is more than half of total cost of hospitalization. 3. Complication: Patients with complications account for 34.20% of all patients, and influencing factors for complications are income, taking exercise, sick time and blood glucose control. Patients with complication spend longer time and more money than patients without complication in hospital.4. Choose hospital-level: 455 patients choose hospital of level 1. 540 patients choose hospital of level 2. 617 patients choose hospital of level 3. Influencing factors for choosing hospital level are income, sick time.Conclusion: Center for community health, patients and general hospital must cooperate to manage diabetes patients. Their tasks include diet control, sports guide, drug therapy, prevention incidence of complication, teaching diabetes knowledge, regular blood glucose monitoring. Patients correctly choose medical institute by policy guidance.
