[Objective] To investigate the effect of the Methadone Maintenance Treatment (MMT) to the heroin addicts, the factors and the compliance to the Treatment.[Methods] A sample of 277 cases of heroin addicts who are willing to receive MMT from October 1 2006 to March 31 2007 in the Methadone Maintenance Treatment clinic in Zhabei District were collected. The horizontal coverage investigation was taken at the grouping and investigation to find out the information of demological characteristics, history of drug abuse, activities of taking illegal drugs, activities of criminals, sexual activities without protection measures and the family's social function etc. Excel software was used for data input and sorting. SPSS 11.0 software was used in descriptive statistics, and multi-factor logical regression analysis was used to obtain the factors of the compliance to the Treatment. Numeration data was used to chi-square test and measurement data was used to rank sum test. The samples are sorted into 2 groups of treatment and drop out according to the condition of the maintenance treatment.20 samples are random selected in each group for further investigation into personal details to analyse the factors of the compliance to the treatment in Zhabei District community.[Results] Among 277 Heroin addicts, the male number is 204, which takes the portion of 73.64% with an average age of 40.69±7.69 years old while the female number is 73, which takes the portion of 26.36%with an average age of 38.23±7.09 years old.66.06% of the total samples were unmarried, divorced or that the spouse had passed away.18.05%of the total samples have junior middle school or lower education.70.76% were unemployed. 68.23% of the total samples recognized that the relationship with their family were in tension. The average age for the male samples to take drugs for the first time was 31.49±8.24 years old, while that of female was 29.75±7.49 years old.275 cases of the samples were heroin taker. The average duration of drug taken was 9.00±4.84 years for these samples. The number of the samples who taken drugs by injection takes is 190, which takes the portion of 68.6%. Among these,14 cases share the injectors together with others which takes the portion of 7.4% of this samples. The seroprevalence of 277 samples was 56.3% for anti-HCV. The seroprevalence was 60.5% for injection drug takers and that was 26.5% for those who take drug orally or by burn and smoke. The former was as 4.25 times as the latter. This result has shown the injecting drugs was one of the main route of Hepatitis C transmission. There is 1 case in the 277 total samples who is attested to have anti-HIV that is male, sino, junior middle school education, unmarried, unemployed, with a history of injecting drugs and also attested to have anti-HCV.Through the comparative analysis of 122 samples in treatment group, the result was shown that the drug acts and injection drug acts after treatment (7.38%) had been reduced obviously compared with those before treatment (100%,88.52%), besides, the high-risk sex behaviors (X2= 13.15, P< 0.05), the rate of crime (X2=5.65, P< 0.05) and the times of relation among friends having drug actions (X2=31.56, P< 0.05) had been reduced obviously compared with those before treatment. In addition, the family relations had also been improved significantly.122 cases (44.04%) of the 277 total samples have better compliance which is be proved by the truth of the times of miss-take Methadone per month are less than 5.155 (55.95%) had dropped out. According to the X2 analysis, there is statistic varience between the treatment group and the drop out group at the aspects of employment, education level, marrige, whether use injection to take drugs, whether share the injector. A multy factor logistic regression analysis shows that:the drop out rate of the employed (37.0%) is lower than those unemployed (68.3%); the drop out rate of the married (34.0%) is lower than those unmarried (58.2%); the drop out rate of the above high school education level (40.0%) and the college education level (18.2%) is lower than those of primary school education level (82.0%); the drop out rate of those who maintain a good relationship with his/her family (20.83%) is lower than those who have a general relationship with his/her family (64.02%); the drop out rate of those who had been injecting drugs (60.9%) is lower than those who does not have injection history (20.6%); the drop out rate of those who share injectors with others (92.9%) is lower than those who have not shared injectors with others (54.0%).According to the personal penetrated investigation, active decline of communication with the partners who takes drugs, getting employed and dip into the work and having good supporting from the family can clearly improve the compliance of the MMT to the heroin addicts. While having been forced to get rid of the drugs in case of slinkingly take drugs and the outings reduce the compliance of the MMT.[Conclusion] Compliance to methadone maintenance treatment among heroin addicts in Zhabei District was relatively low. More efforts are needed to promote such complicance and to minimize drop out.
