Study On Risk Factors Of Hepatocellular Carcinoma In Kaifeng County, Henan Province

Backgrounds and ObjectiveHepatocellular Carcinoma(HCC) is one of the most common human cancers, which has about 1,000,000 new cases annually in the world,and it causes great harm to people's health.The geographic distribution of HCC is remarkable around the world.Kaifeng,a rural county,is a high incident area of Henan for HCC in China. Based on the data from Kaifeng Cancer Registry,the annual average death rate of male was 31.32/100,000 and of female was 13.44/100,000 during 16 years from 1987 to 2002.In order to find out the relative dangerous factors of HCC,the study used the case-control epidemiological methods which were population-based,it also introduced the logistic regression analysis.These methods comprehensively evaluate the function of the dangerous factors on HCC,and these provided a theory for preventing and controlling the happening of HCC.Meterials and MethodsCases:Choosing County Usual Residents who got HCC from September 1,2006 to August 31,2007.The cases ruled out those patients,whose HCC were transferred from other parts of the body.All the cases based illness history,clinical performance, the B-ultrasound,CT and the various indicators of laboratory examination,diagnosis of HCC,some cases were confirmed by the pathological diagnosisContrast:In accordance with the individual matching principle,the age space of the compared pairs can not more than 3 years.They were matched by 1:1,patients neighbors were selected randomly,they met the conditions,there were 149 couples forming the sample according to the design of individual matching.Investigation:Demographic information(age,gender,occupation,education, marriage,etc),the basic situation of cases,smoking(smoking or not,smoking age,quantity,etc),alcohol drinking(drinking or not,drinking age,quantity,etc),tea drinking(drinking or not,drinking age,quantity,etc),water drinking,food(including the usual eating habits and food intake,and so on),mental factors,physical activity, harmful exposure history and family history of cancer,such as a total of more than 100 indicators.In this study,all the data was analyzed with the statistical software of SPSS12.0 package,the univariate factor logistic regression and multi-factor conditiona stepwise logistic regression were used to analyze different faetors's impact on HCC.So as to calculate the OR and 95%confidence interval of the factors.Results1 Univariate factor Logistic regression analysisThe results showed that the number of monthly alcohol drinking(OR=5.36,OR 95%CI:1.95～14.70),personality types(OR=1.72,OR 95%CI:1.04～2.84),angry (OR=11.53,OR 95%CI:5.27～25.26),major mental stimulation or major trauma (OR=3.00,OR 95%CI:1.47～6.10),self-illness,accidents,not suitable to work, long-term insomnia,hard working and physical strength,hepatitis disease history,the history of other diseases(OR=3.03,OR 95%CI:1.83～5.02),and tumor diseases (OR=5.33,OR 95%CI:2.27～12.54) were the HCC dangerous factors.Extrovert characteristics(OR=0.59,OR 95%CI:0.37～0.94)was the HCC protective factor.2 Conditional Logistic regression analysisThe relationships of the factors were synthetical analyzed in the study,using the conditional logistic regression analysis.The results showed that angry(OR=14.13,OR 95%CI:3.07～65.00),diseases(OR=19.74,OR 95%CI:2.68～145.64) family diseases history(OR=IS.47,OR 95%CI:3.05～111.85),family cancer history(OR=309.67,OR 95%CI:8.42～11385.05) and so on were the dangerous factors of HCC.Since the compared crowd had not patients with a history of hepatitis,on the one hand,it shows that the patients with a history of hepatitis belong to the high-risk groups.On the other hand,it shows that there may be some impartial investigation,the introduction of the hepatitis history cause regression coefficient bias.Therefore,this information logistic regress models did not introduce the factor of hepatitis history.ConclusionsThe results of this study showed that:HCC was interrelated with many factors, especially the patients with a history of HCC were the high-risk groups,in Kaifeng County.Getting angry easily,immediate family history of disease,family history of cancer had important roles in the incidence of HCC,particularly getting angry easily may be a potential intangible factors.So,Dynamic surveilance and comprehensive prevention and control of the factors of HCC in Kaifeng County should be more strengthened,especially the patients with a history of HCC should be timely treatment, rational nutrition and balanced meals,science diet and kept a good mood to take precautions against HCC.
