Comparison Of Total Knee Replacement With And Without Patella Replacement

Objective:To compare retrospectively the clinical outcome of total knee replacement with and without patellae replacement.Methods:From April 2006 to April 2010.60 knees in 56 patients treated for osteoarthritis evaluated postoperatively using a questionnaire and physical examination. There were 20 males and 36 females with the age ranging from 59 to 80 years (average 62.5 years). The osteoarthritis was all knees,28 knees underwent patella replacement, and 32 knees with reserved patella. The patients were scored using HSS Score for knees and Feller Score for patella. AP and lateral views of the knee as well as 90°axial views of the patella were taken in radiography. The data were analyzed using SPSS15.0 software.Results:Replacement and non-replacement groups showed obvious difference in post-operative knee the incidence of complication. Replacement group have little moving than non-replacement group. The two groups showed statistical significant differences in scoring on climbing and descending stairs and rising from sitting position, the results suggested the non-replacement group of patella function scored slightly better than the replacement group in post-operative(p<0.05). Post-operative anterior knee pain occurred less commonly in the two groups and showed no significant difference (p>0.05) in the two groups. All patients had not any complications, for instances:infection dislocation,fracture and so on.Conclusion:1. No matter if replacement of patella, improve patellofemoral' track that it is a important factor about reducing post-operative knee the incidence of complication and anterior knee pain.2. If patellofemoral'track and force line of knee are excellent osteoarthritis'patients are not replace patella in routine.It is a significant of knee'fouction of recovering that reserved patella.
