Objective Critical disease is status of systemical inflammatory response ( SIR ) of body under stimulative factors. SIR is relative to inducing and expression of some of cytokines such as ET and TNF . To research the changes of ET and TNF in critical disease provided new thinking ways to investigate pathogenesis and therapy approaching in critical disease. Methods The concentration of serum of ET and TNF of 20 critically ill patients, 18 non-critically ill patients, 10 rats with endotoxic shock and 10 normal rats were tested. Results Results showed that the serum levels of ET and TNF of both critically ill patients and rats with endotoxic shock were obviously higher than those in the controlled groups, the serum levels of ET and TNF of critically ill patients with MOF were higher than those with single organ failure and non-organ-failure groups. Meanwhile, it was significantly higher in disease worsing group than it in curing and improving groups. Conclusion Entoxin is an important inducer of TNF. As a frigger of SIR, TNF activates the network of cytokines, inducing SIR and take parting tissue injuring with ET and possibly leading to MOF. The serum levels of ET and TNF are related to states of organ functions in critical disease developing. Therefore, it can be an alarming indicator of critical disease with MOF, it also can be a predictor of prognosis and outcome of critical disease.
