Chest blocking and heartache is one of common clinical syndromes. It results from heart-meridian blocking induced by Qi-deficiency, phlegm, coagulated blood, Qi-stagnation and coldness-attacking which is clinically manifested as paroxysmal compression feeling and pain in the chest or Tanzhong. Its incidence tends to increase gradually with the change of lifestyle and dietary structure in the modern society. It is one of diseases and syndromes of the heart system which endanger the health of middle and old people. It has complicated manifestations and pathological change. Traditional Chinese medicine and pharmacy therapy is widely concerned as its predominance over comprehensive action resulting from its taking the whole body into consideration.Many doctors and clinicians gradually consummate its onset law and main points of syndrome differentiation and summarize comprehensive experience of clinical administration and accumulate plenty of relative literatures during their clinical observation and practical recognition over two thousand years from the pre-Qin dynasty to the end of Qing dynasty. Historical origin and development and special advance in each dynasty was narrated in the literatures of TCM from pre-Qin to the end of Qing dynasty and characteristics in different phase was retrospected and studied in this article which will be beneficial to make profound exploration on the treatment and syndrome differentiation of chest blocking and heartache in the later times.Two parts were included in this article. The contents of the first and second chapter is the first part which determined the definition and range of chest blocking and heartache in this article and made its clear definition according to the standard of modern Chinese medicine and made certain the range of diseases and disorders in western medicine included in the chest blocking and heartache and other diseases manifested as similarly main symptoms were also included in the same range. As many relative diseases name of this syndrome existed in the ancient literatures and different knowledge occurred in the different dynasty, meaning and its origin and deviation of ten names related with chest blocking and heartache was explored and discussed in the second chapter. Owing to the limited situation, heartache in the internal medicine was discussed in the article. Heartache and bellyache in febrile diseases, pregnant heartache, heartache after partus, children heartache and cholera heartache etc were excluded from discussion in this article.Academic origin and development in those aspects of etiology, pathogenesis, diagnosis, treatment and nursing etc were explored and discussed in the second partdivided into the following phases as pre-Qin, Han-Jin, Shui-Tang, Song-Jin-Yuan,Ming and Qing dynasty. It is rudimentary recognition phase for the syndrome of chest blocking and heartache in the Qin-Han dynasty. In this phase, physiology, manifestation, prognosis and acupuncture and moxibustion therapy about this disease were narrated to some extent in the Huangdi's Internal Classic; the name of chest blocking was put forth and its etiological factors and pathogenesis and medicinal treatment were systematically expounded in the Synopsis of Prescription of the Golden Chamber(Jin Kui Yao Lue) which made foundation for the development of theoretical system of syndrome differentiation and treatment for this diseases and syndrome later. Chest blocking and heartache syndrome was distended and developed in the phase from Shui-Tang to Ming Qing dynasty. Knowledge was gradually deepened and therapeutical experience was enriched and theoretical system was consummated with the development of society, economy and culture in this phase. Treatment for heartache was found in the prescriptions books but literatures about systematic treatment were scarce in the phase of Tang-Song-Yuan dynasty.
