| [Object]: To evaluate objectively the changes in the ocular refractive status after retinal detachment (RD) surgery , and to analyze not only the correlation between operative pattern and refractive status, but also effect of different operative pattern upon refractive status.[Methods]: 29 eyes of RD with successful encircling scleral buckling surgery(SB) and vitrectomy underwent Zywave system, Orbscan II system and A-scan, The refractive factors of both groups pre- and post-operation was observed including the depth of anterior chamber, thickness of lens, axial length of eye, simulated keratoscope reading (SimK), astigmatism, mean power of corneal central curvature(3mm), diopter, best corrected visual acuity (BCVA). And wave-front aberrations of twenty-two eyes of 11 post-SB patients was investigated .[Results]: Root-mean-square (RMS) of the 3rd ,5th and overall higher-order aberrations in 11 post-SB eyes was 10.271, 0.327 and 0.809, comparing with 11 normal eyes, RMS increased averagely 1.426, 1.496 and 1.229 times respectively, the differences were significant, and that of 4th aberrations had no statistic significance. Wave-front aberrations could not be got in vitrectomy group because of diopters were over -12D or/and being filled with silicone oil. There was significant difference between pre-SB and post-SB in Kl, astigmatism, axial length, the depth of anterior chamber, thickness of lens, maximal protrusion values and diopter. The state was opposite in the 7 eyes in vitrectomy group eliminating of the other 3 cases which with silicone oil Filling. There was no significant difference in K2, mean power of corneal central curvature (3mm) and BCVA of SB group, also in all parameters of vitrectomy group except of maximal protrusion values. In SB group, Diopter significantly correlated with axial length and Kl, the correlation ship between diopter and refractive status in vitrectomy group could not be get because of the small quantityof sample .[Conclusion] : The 3rd ,5th and overall higher-order wave-front aberrations increasedpositively after SB surgery, Lower-order aberrations such as spherical and cylindroiderrors increased significantly due to axial length and corneal curvature.
