Objective To facilitate the diagnosis and treatment of intrathoracic sarcoidosis. Methods The clinical data of 18 cases of intrathoracic sarcoidosis confirmed by pathology during the years of 1992 —2005 were retrospectively analyzed. Results Among these patients, 61.1% cases showed symmetrical enlargement in the bilateral lung hilus. 73.33% cases had increased serum angiotensin-converting enzyme levels,and 83.33% cases showed negative result in purified protein derivative skin test. Conclusion Intrathoracic sarcoidosis has atypical clinical symptoms. BMB, TBLB and lymphonodi biopsies are the convenient methods with highly diagnostic and less invasive values to the diagnosis of intrathorasis sarcoidosis. The diagnosis should be reached by a careful evaluation of the clinical representation in conjunction with X-ray, sACE and PPD test results.
