Abstract/Summary:
Objective To gain the new CIRindex and correlation of CIRindex with other factors about insulin pump with adjusting basal rate and prandial boluses.And to study basal rate and the proportion of basal rate to TDD(total daily insulin dose,TDD).Methods Thirty eight diabetes mellitus patients(17T1DM,21T2DM,22M,16F,aged 43.73±19.11,BMI 24.10±3.22kg/m2,duration of diabetes 7.74±6.09 year,duration of insulin pump therapy 2.04±0.98 year,HbA1C8.16±2.05%)that were treated by insulin pump were selected as studied objects.Firstly,to evaluate basal rate without meal and adjust it exactly to make FBG within 4.4~6.1 mmol/L and only BG cutting down lightly(<1.7mmol/L)without meal.secondly,to formulate the standard meal which it is steamed bread weighting 133g and containing 60.8g CHO(carbohydrate,CHO).To adjust prandial boluses exactly to make 2HBG<7.8mmol/L and get pre-prandial BG after post-prandial 4 hours.Lastly,to calculate the best CIRindex according to the best basal rate and prandial boluses and study the correlation of CIRindex with other factors,basal rate and the proportion of basal rate to TDD.Results There was a significant difference between the new CIRindex and the pre-CIRindex (P<0.001).Basal rate was less than 50%of TDD(P<0.05).Basal rate and the proportion of basal rate to TDD were correlated with the new CIRindex(r=0.552,P<0.001 and r=0.958,P<0.001 respectively).Conclusion The new CIRindex and something about basal rate play an important role in insulin pump therapy.These results may supply a new guide to treat diabetes mellitus by insulin pump.The correlation of CIRindex with insulin resistance andβ-cell function would be used in insulin pump therapy.To point out the different index about the medical science in the different region and race.
