Senile dementia (SD), with much higher morbidity among old people, usually leads to low quality of daily life and high expanse of healthcare, and further brings on social public health problems and economic burden. There are no better solutions yet now. Some studies indicated that TCM is significantly effective on SD in both prevention and treatment; however, the related evidence is hard to be found out in the large number, of literatures. The large amount of literatures provides data and target of TCM sorting and understanding and treating SD. The application of modern scientific methods to these sources is to be paid attention to in TCM research. The sorting of classical literatures review tells us that the key pathogenesis of SD is ben-deficiency and biao-sthenia. Ben-asthenia refers the kidney deficiency resulting in the insufficiency of brain marrow; biao-sthenia lies in blood stasis and phlegm blocking the brain collaterals. This article is to sort the historical academic ideas and theories for it and concentrate on related literatures in the near 27 years for a SD database based on the previous study with literature sorting, statistical processing, and logical analysis. The 90 basic prescriptions with related 140 drugs for SD were sieved out from 489 articles according to the collecting principles. The comparison between the results of historical and modern literatures was to be carried out so as to understand the pathogenesis, syndrome and treatment of SD.The study on the literatures of recent 27 years indicates that the first three frequently applied drugs are Shi Changpu (Rhizoma Acori Tatarinowii), Shu Dihuang (Radix Rehmanniae Preparatae) and Dan Shen (Radix Salviae Miltiorrhizae) with the frequency rate are 75.56%, 63.34% and 60% separately. They are of eliminates phlegm for brain orifice, reinforcing kidney and essence, promoting blood flow and removing blood stasis categories respectively. The application of drugs shows that the first three categories of all are those with replenishing deficiency function, activating blood circulation and removing blood stasis function as well as eliminating phlegm function with the rate of 71.2%,64.3% and 60.1%. The first three kinds form many specific pair. The first three meridians the drugs act on are the liver meridian, the kidney meridian and the spleen meridian. All the results are of significance after statistical processing (to see the P in the article). The drugs applications of SD are: the drugs with replenishing action, activating blood circulation and removing blood stasis and phlegm for brain orifice action are the frequently applied kinds in light of its main therapeutic principles of replenishing the liver and kidney, activating blood circulation and removing blood stasis and phlegm, tonifying the spleen. This is the basic law of SD during 27 years of inheritance TCM theory and practice.TCM holds that SD due to blocking occurs in the brain with its origin in the kidney, indicating that the pathogenic factor is phlegm. It is said that "the dementia is severe if the phlegm is superabundant". What's more, the tingered SD is always complicated with blood stasis; therefore, doctor of the past usually treated it from the aspect of asthenia, phlegm and blood stasis. This research focus in drugs of SD, but it reveals its basic therapeutic principles and methods are to reinforce the liver and kidney, activate the blood to remove stasis, strengthen the spleen to eliminate the phlegm. The therapy proved that the basic pathogenesis is above for SD and suggests that the TCM theory on SD is clinically correct.Tradition believes that the occurrence and development of SD are closely correlated with the kidney and heart. Few people protest the importance of the liver. But the meridian action result showed that the liver meridian action drugs were often used in clinic. It might relate to the relationship between the liver and the kidney as well as the wind syndrome which should be paid more attention to. The result at least suggests that the liver be studied on its role in SD.On the whole, the basic therapeutic principle for SD is to reinforce the kidney and essence and removing the phlegm and blood stasis, which is promising according to the pathogenesis of SD "kidney-deficiency and essence consumption as ben-asthenia, and blood stasis and phlegm as biaosthenia". Simultaneously, system of syndrome differentiation and treatment is to be established on the basis of criterion of different TCM syndrome types with the combination study on TCM syndrome and disease of SD. By the same time, the modern molecular biological techniques, the target function, pharmacological mechanism of "reinforcing kidney and eliminating phlegm and blood stasis" action are all to be studied so as to find out the effective TCM prescriptions for SD and it is meaningful to interfere in the occurrence and development of SD.
