| ObjectiveTo discuss the operative indications and choice of dynamic hip screws and proximal femoral nails in the treatment of intertrochanteric fractures of the femur in elderly patients, and compare clinical effects of the two fixation.Methods:We treated 60 cases of intertrochanteric fractures of the femur from 2004. 9~2006. 3 in the traditional Chinese medicine hospital of Hubei province, the Wuhan General Hospital and Shiyan Taihe Hospital. 33 cases were treated with dynamic hip screw combined with screws and 27 cases with PFN, all the preoperative, intraoperative and postoperative datas of the 2 groups, including Operative duration, Length of incision, Intraoperative blood loss, Postoperative drainage, Femoral neck collapse cases, Coax vara cases, were observed and analyzed statistically.Results:All the cases were followed up for 12 to 24 months, 18 months in average, and all the fractures healed. The incision length, operation duration, intraoperative blood loss, postoperative drainage and complications were statistically different between the 2 groups(P<0.05). The forepart time of siting up, Hospitalization period, and time of fracture healing between two groups have no statistical difference(P>0.05).Conclusions:The patients with femoral intertrochanteric fracture should be operated. PFN internal fixation is more suitable for the elderly patients because it has less operative trauma and fewer postoperative complications, so it is relatively an ideal implant for the treatment of femoral intertrochanteric fracture at present. But the operation indication should be chosen strictly and the propotional treatments should be taken during operation according its characteristic and positive prophylaxis should be taken to educe the occurrence of complications. |
