Hip Bone Marrow Edema And Osteonecrosis Of The Correlation Study

Objective To explore the relationship between the serious degree of bone marrow edema (BME) of coxa joint and osteonecrosis of the femoral head (ONFH), including the type of traditional Chinese medicine (TCM), Harris score and collapse of the femoral head. This can help to further recognize BME in ONFH, and guide the pathogenetic condition and prognosis.Method According to X-ray and MRI, retrospective study the relationship between the class of BME and grouping of TCM on 69 patients (121 coxas) who have been final diagnosed as ONFH accompanied BME, then retrospective study the relation on the class of BME, whether femoral head collapse or not, and the Harris score on 58 patients (94 coxas), chose from the previous 69 people.Result 1 ONFH with BME was light in the early stage,most of which are the 1st and the 2nd class; while it was serious during intermediate stage and most of them are the 4th class; afterwards BME dissipated and most of them are the 1st and the 2nd class in the terminal stage;2 The collapse situation and rate are as follows: 4.5% in the 1st class, 11.8% in the 2nd class, 66.7% in the 3rd class, and 90% in the 4th class;3 Our finding reveal that there is a marked negative relation between the degree of bone marrow edema(BME) of coax join and Harris score:93.0±5.41 in the 1st class,84.1±5.42 in the 2nd class,76.4±4.22 in the 3rd class,66.3±7.46 in the 4th class, the more serious BME, the lower Harris score are.Conclusion1 BME is the concomitant sign of ONFH development; The degree of BME developed progressively with ONFH during early and middle stage before femoral head collapse. BME dissipates gradually with the collapse of the femoral head and formation of osteoarthritis in the terminal stage; The studies have also shown that the classification of BME can be used as the evidence of ONFH by TCM symptoms differentiation and treatment;2 Through the serious degree of bone marrow edema, we can forecast the collapse of the femoral head in other way;3 There is a marked negative relation between the degree of bone marrow edema (BME) of coax join and Harris score, in other words, the more serious BME, the lower Harris score are.
