Shoulder injury is a common disease in acupuncture, massage, orthopedics, and rehabilitation out-patients departments. It can happen at all ages, particularly middle-aged and elderly most, including subacromial syndrome(shoulder impingement),rotator cuff tears and adhesive capsulitis etc.A high incidence of the disease,it has repeatedly deferred to the patients suffering great, but also a great impact on the patient's social activities and quality of life.Literature Review Partâ… :Shoulder injury of Chinese and Western medicine for diagnosis and treatment of recent literature are reviewed. In recent years, the diagnosis of shoulder injury is not simply a general "frozen shoulder" concept, but increasingly refined.Through the symptoms, signs and laboratory, diagnosis of shoulder injury is increasing clear. Both Western and Chinese medicine for the shoulder injury are more inclined to conservative treatment, and only some sportsmen and patients who failed to respond to conservative management select operations. In fact, most patients with shoulder injury select conservative treatment, achieving a good therapeutic effect.Joint mobilization, and acupuncture at ST38(Tiaokou) are widely used in clinical.A number of literatures report multi-therapy is better than a simple method. However,it may take 2-3 more times combining different therapies than a single treatment,or even longer.Whether we could save treatment time as little as possible become an issue.Clinical Research Partâ…¡:Objectives:This study seeks to evaluate the effectiveness of joint mobilization applied with a treatment of acupuncture at ST38(Tiaokou) for shoulder injury. Methods:A study was made of 20 patients presenting symptoms of painful shoulder with or without shoulder activity limited, and a diagnosis of subacromial syndrome (shoulder impingement) rotator cuff tears and adhesive capsulitis etc.The patients were randomized into two groups:1) experimental(joint mobilization + acupuncture);2) control (joint mobilization). The treatment took 4 weeks. The result variable was a JOA shoulder scale.Following examinations would be made at 4 and 8 weeks after that. Results:Both two treatments could effectively relieve shoulder pain and improve ROM of the shoulder joint, further more increasing shoulder activity. For the high efficiency, joint mobilization with a treatment of acupuncture at ST38(Tiaokou) was confirmed much better than using only joint mobilization. Conclusions:The treatment of acupuncture at ST 38(Tiaokou) can relieve pain immediately,to facilitate the implementation of joint mobilization while reducing spasm to enhance the efficiency of joint mobilization. It is more valuable that as a point located at leg,ST38(Tiaokou) makes it possible to implement acupuncture and joint mobilization at the same time. This method overcomes the loss of effect when doing the joint mobilization after pulling out the needle.Joint mobilization applied with a treatment of acupuncture at ST38(Tiaokou) can obviously improve the curative effect of subacromial syndrome, and shorten the period of treatment. It can greatly economize medical resource.
