| Objective To investigate the clinical character and effective treatment of each type of benign paroxysmal positional vertigo(BPPV) and the results of treatment by appropriate repositioning maneuvers.Methods 275 patients with BPPV treated between March 2007 and December 2008 were recorded with history,etiological factor,motivation,cause,diaognosis,the positioning nystagmic features,treatment,frequency of instant treatment,re-examination after one day,seven days and three months.To assess the efficacy of canalith repositioning procedure (CRP) in the treatment of patients with BPPV,including curing,amelioration,inefficacy and palindromia.Results Of 131 patients,posterior canal involvement were randomly divided into 2 groups and treated with canalith respositioning procedure and Semont liberatory maneuver, respectively.Their effective power one day later were both more than 90%,seven days later more than 95%。And the former group was higher than the later one.According to theχ~2 analysis,the two had no significant statistical deviation.At the same time,the effective power of the control group was lower than the former two groups.Comparing it with the former two groups,it had significant statistical deviation.Reccurence of the three groups was like this,control group (20.0%)>Semont group(9.5%)>Epley group(6.5%).The effective power of age group were more than 85%one day and seven days later,74.1%three months later.On subjective group was like this,next day(74.3%)>seven days later(65.7%)>three months later(62.9%).While,the effective power of horizonial canal involvement was 80.0%next day,more than 90%seven days later and three months later.And anterior canal involvement was100.0%,respectively.Conclusion BPPV,clinically characterized by transient vertigo and nystagmus evoked by switch of head position is caused by many factors.The Positioning maneuver is the golden key to the diagnosis.In many treatments,Canalith repositioning procedure is widely used..After the BPPV types and the affected side were determined,the appropriate repositioning maneuvers were chosen and performed.Epley and Semont maneuvers were both effective to diagnosed posterior canal involvement.That the effective power were both more than 90%,much higher than that of common drugs,while reccurence quite converse.However,those without final diagnosis or the positioning nystagmic features,had lower effective power,which grew lower as time went by.To horizonial and anterior canal involvement,they also can get great efficacy. Many patients of these disorders can remit immediately after once or twice Canalith repositioning procedure.Our data suggest that the canalith repositioning provides safe and effective treatment in the majority of patients with BPPV,and was in fact responsible for the resolution of symptoms at clinic service. |
