Palate The Pharyngoplasty Clinical Path Of Applied Research

Objective:The paper intended to establish the perfect clinical paths of Uvulopalatopharyngoplasty.The research has explored the way for the Uvulopalatopharyngoplasty's clinical pathway corresponding to the actual situation in China,has exerted the practical significance in enhancing the medical quality,specifying the process of diagnosis and treatment,shortening the hospital days,lowering the medical cost, advancing the reasonably utilization of medical resources,and has provided experience for making and practicing clinical pathway of other diseases.Method:In this research,retrospective method was adopted.First,with the aid of hospital information system(HIS),we retrieved the information on Uvulopalatopharyngoplasty patients from June 1^st,2007 to February 29^th,2008.Second,according to the theories and methods such as Evidence-based Medicine,Vilfredo Pareto Principle,Delphi Method, Median and so on,the research designed the hospitalization time,contents and items in diagnosis and treatment flow of clinical pathway,which were put into practice in clinical department in March,2008.After a period of practice(7 months),systematic analysis and evaluation of the data by the hospital information system were conducted.Result:There are 53 patients in clinical pathway group and 53 patients as control group drawn from 89 patients in traditional group which amount to be 106 patients in total. There are no significant differences between the two groups in terms of age and sex.First, the hospitalization time,preoperative waiting time,days of injecting antibiotics intravenously in clinical pathway group are shorter than that in traditional group and there are significant differences between the two groups.Second,total hospitalization expenses and each item expense of hospitalization are less than that in traditional group and there are significant differences between the two groups.Third,there are significant differences between the two groups on the degree of patients-satisfaction.The last,there are no significant differences between the two groups on convalescence rate,diagnostic coincidence rate of discharging and adimitting,the first degree medical record rate.Conclusion:The clinical pathway is an advanced model for the control of medical quality,which does not weaken care effect,and significant shorten average lengh of days, and lower the average hospitalization cost,promote reasonable utilization of medical resources.Meanwhile,it has also atual effect for clinical pathway on advancing normalization of diagnosis and treatment,promoting the co-operations among medical staff and the communications between medical staff and patients.In addition,the practice of clinical pathway also revealed that the traditional habits,the current running system and service idea were not able to meet the requirements of the complete practice of clinical pathway.Therefore,clinical pathway requires that hospital administrators should into full play.
