This project tried to investigate the relationship between the degree of disease severity and chronic obstructive pulmonary disease with acute exacerbation of the syndrome factor of TCM through document review and clinical research.Document review:The review of traditional chinese medicine documents included pathogenesis of TCM, syndrome differentiation, treatment method on chronic obstructive pulmonary disease with acute exacerbation. Also discuss the relevance between type of TCM syndrome on COPD and modern medicine objective indicators. including lung function, blood gas analysis, immune, blood, biochemistry and so on.The review of westen medicine documents included the definition of COPD, risk factors, pathogenesis, diagnosis, standards of stage and grade and treatment and so on. Clinical research:Objective:Study relationship between the degree of disease severity and symptoms of traditional Chinese medicine elements of Acute Exacerbation Chronic Obstructive Pulmonary Disease (AECOPD)。On the one hand take the lung function graduation as the standard, study the evolution rule of the internal organs pathogenesis of COPD. In order to master the ins and outs of the disease and pathological changes in the law, combined with the different organs disease-bit selection guide for drugs, thus enhancing targeted therapy. On the other hand, discuss the association between the TCM syndromes of COPD and the degree of disease severity, attempts to establish a research methods of the clinical syndrome model, that is experimental to provide a diagnosis objective and quantitative index for the evolution of COPD in the pulmonary function test results in order to be able to objectively diagnose the TCM syndromes of COPD.Method:A retrospective analysis is adopted. Clinical cases that meet diagnostic standard by western medicine use four diagnostic methods to be grouped on symptoms of Traditional Chinese Medicine.Then refer to Academician Wang Yongyan'thinking of complex"syndromes"to reduce the dimensions, the complexity of each patient's"syndromes"decomposition as a single disease location and nature of the"single card"to extract the elements of disease location and nature. Then they are classified according to the standard classification of lung function and found that the correlation between them.Results:Location of disease:In the four stages of lung function, the lung lesions occur most frequently, As condition of the disease increased, increasing the level of lung function, complex disease-bit also gradually increased, mainly lung and kidney, lung, kidney and heart. Spleen and kidney. Location of disease involved in the lung, spleen, kidney, heart and lungs, spleen, stomach, kidney, liver became fewer. Lung functionⅡ,Ⅲgrade has high correlation with lung, spleen. Lung functionⅣhas high correlation with kidney.Nature of disease:In the positive syndrome, the phlegm-syndrome appears most frequently, followed by heat syndrome and blood stasis. In theⅡ,Ⅲlevel, the phlegmy heat syndrome accounted for the first. Syndrome of intermingled phlegmy heat and blood stasis gradually increased, In theⅣlevel, Syndrome of intermingled phlegmy heat and blood stasis appeared more frequently than before. InⅡ,Ⅲ,Ⅳlevel, qi deficiency accounted for more than 50% of cases. Yin deficiency accounted for 4.9%,4.9%,5.1%. Lung functionⅡhad high correlation with phlegm and heat. Lung functionⅢhad high correlation with heat and qi-deficiency syndrome. Lung functionⅣhad high correlation with blood stasis.Conclusion:Location of disease was located in the lung first, As the disease increased, disease-bit gradually involved the spleen and kidney. In the positive syndrome, the phlegmy heat syndrome accounted for the first. With the increasing severity of disease, the number of the deficiency and blood stasis syndrome gradually increased. In the deficiency, qi-deficiency syndrome is in the first.
