According to the world health report 2000 published by the WHO, China ranked 188 among the 191 member countries in terms of the medical contribution equality. In China, the resource of medical security is unequally distributed with more than 2/3 resources given to the 36% members in city while the 1/3 left given to the peasantry, about 64% in all. Further more, about 87% rural members in China are out of the security of social medical security, and the poverty and return to poverty due to disease have become a widespread phenomenon which has seriously affected the continuous development of our country. Therefore, how to select the rural medical security system and how to built it has become two serious projects which should be treated appropriately and in time. China's rural cooperative medical security system, which was once successfully recommended to other developing countries in the 1960s and 1970s, has been disabled in the 1980s, and about 90% rural members in China have been lack of medical security since 1990s. In 2002, Chinese government promised that the new rural cooperative medical security system will be established by 2010. So far, in our country, the studies on rural medical security system have been paid little attention, and mainly center on issues of cooperative medical security system, such as funds raise, medical treatment expenses compensate and control, etc. but be lack of the synthetic analysis on hygiene epidemic prevention, medical service, medical security as well as economy and society. This thesis regards the basic analysis and the synthetic analysis as the fundamental point to study the rural medical security system. This essay proceeds as follows: Firstly, after brief introduction of the development of rural medical security and its present situation, as well as some major problems encountered by rural cooperative medical security system, the necessity, feasibility and conditionality of establishing the multi-layer rural medical security system are analyzed from various aspects such as rural economic development, changes in social structure, reforms of rural medical treatment system, tradition and consuetude, etc. Secondly, after studying foreign models and experiences, a new multi-layer rural medical security system is established, including basic medical security system, cooperative medical security system, supplementary medical security system. Lastly, some suggestions on establishing the new multi-layer rural medical security system are brought forward. This essay is divided into five chapters: Chapter One firstly retraces the history of rural medical security system and analyzes the ups and downs of the first and second cooperative medical security system respectively. Secondly, this chapter analyzes the present situations of rural medical security and the major problems encountered in the present operation by rural cooperative medical security system. Chapter Two systematically analyzes the necessity, feasibility and conditionality of establishing the multi-layer rural medical security system in China today, according to such aspects as rural economic development, changes in social structure and peasantry income, reforms of rural medical treatment system, population flow, tradition and consuetude, etc. Chapter Three introduces Korea National Health Insurance and Non-profit insurance schemes for the unorganized sector in India firstly, and then gives some proposals to reform and perfect the rural medical security system.Chapter Four gives a new multi-layer rural medical security system which includes basic medical security system, cooperative medical security system, supplementary medical security system. The new multi-layer rural medical security system is a systematic design and a new system under the specific political, economic, social and cultural circumstances of China, from both practical and theoretical angles.
