Study On The Serum Ghrelin And Leptin In Patients With Gastroesophageal Reflux Disease

Objective:GERD is a common digestive disease and its pathogenesis is complex and can not be very clear. Ghrelin is a growth hormone that is releasing peptide consisting of 28 amino acids, Leptin is a hormone secreted by fat cells and its express is regulated by obesity gene. They play a importment role in gastrointestinal tract motility and, are involved in feeding. Serum Ghrelin and Leptin level were detected in patients with GERD to explore pathogenesis of GERD, and provides a new approach for its clinical treatment.Methods:Sixty one patients with GERD were divided into two groups:RE (Reflux esophagitis) group (25 cases, male 11, female 14; aged 23-75); NERD (Non-erosive reflux disease) group (36 cases, male 17, female 19; aged 21-80). Twenty six healthy people were selected as normal control group(male 12, female 14; aged 26-79). Serum Ghrelin and Leptin level were detected in patients and normal control by double antibody sandwich enzyme-linked immunosorbent adsorption method (ELISA), and then analyse the result statistically.Results:1. The serum Ghrelin level was 15.48±2.76 (ng/ml) in GERD group,58.52±10.77(ng/ml) and 60.74±11.00(ng/ml)in RE and NERD, but 70.43±11.07(ng/ml) in normal control group. The serum Ghrelin level in GERD group was significantly lower than that in the normal control group (p<0.01). The serum Ghrelin level in RE and NERD was significantly lower than that in the normal control group (p<0.01).2. The serum Leptin level was 15.48±2.76 (ng/ml) in GERD group,15.38±2.94 (ng/ml) and 15.55±2.67 (ng/ml) in RE and NERD, but 13.81±2.74 (ng/ml) in normal control group. The serum Leptin level in GERD group was statistically higher than that in the normal control group (p<0.05). The serum Leptin level in RE group was statistically higher than that in the normal control group (p<0.01), and the serum Leptin level in NERD group was also statistically higher than that in the normal control group (p<0.05). 3. There were negative correlation between serum Leptin level and serum Ghrelin level in patients with GERD (p<0.05). There were also negative correlation between serum Leptin level and serum Ghrelin level in RE group and NERD group (p<0.05).Conclusion:1. The serum Ghrelin in patients with RE and NERD was decreased, which may cause delayed gastric emptying. The serum Leptin in patients with RE and NERD was increased, which may be related to inhibition of gastric emptying. It is showed that the alteration of serum Ghrelin and serum Leptin may play a role in the pathogenesis of GERD.2. In the patients with RE and NERD, the serum Ghrelin and Leptin have negative correlation. It is showed that the serum Ghrelin and Leptin may have opposite effect on each other in the pathogenesis of GERD.
